When does adapalene make sense?

Adapalene is worth discussing if acne is also a concern: it helps prevent spots and reduces inflammation and oiliness. If wrinkles are your only concern, ask about tretinoin instead. Adapalene is generally the least irritating topical retinoid, while tretinoin is the most irritating, so tolerability matters alongside the treatment goal.23

In the US, adapalene cream is a prescription formulation; adapalene sold without a prescription comes as a gel. The word cream describes the format, not a contractual obligation to make you younger. Creams are generally less irritating than gels, but adapalene cream can still cause dryness and burning.124

What results should you expect?

Fine lines and sagging skin are different targets. Retinoids can help the skin make more collagen, but a cream cannot lift sagging skin. Moisturizer can temporarily plump skin and make lines less noticeable, which is useful even if the bathroom mirror has mistaken hydration for renovation.5

For acne, adapalene can take 8 to 12 weeks or longer to deliver its full benefit. Do not use that as a deadline for wrinkle improvement: an adapalene cream wrinkle timeline remains uncertain. Tretinoin can reduce fine wrinkling with long term use beyond six months.24

How do you use adapalene cream?

  1. 1Check your prescribed schedule. Adapalene is usually used once daily at bedtime. General retinoid guidance suggests starting every other night and building up slowly; agree on any change with your prescriber.
  2. 2Wash your hands, gently cleanse with a mild or soapless cleanser, and pat the skin dry. Do not scrub.
  3. 3Spread a thin film over the area your prescriber tells you to treat. Keep it away from the eyes, lips and nose creases. Wash your hands afterward.
  4. 4Use moisturizer for dryness. During the day, use sunscreen, protective clothing and shade, and avoid tanning beds.

Applying more cream or using it more often will not improve results faster and can irritate your skin. If you miss an application and the next one is almost due, skip the missed application rather than doubling up.47

What should you avoid alongside it?

For the US prescription adapalene 0.1% cream, avoid products containing alpha hydroxy acids, including glycolic acid. Salicylic acid, abrasive cleansers and strongly drying products also need caution. Do not add other acne treatments to the same area unless your clinician directs you to use both. Tell them about prescription skin medicines, including topical antibiotics. Avoid waxing treated skin, and never apply adapalene to broken, sunburned or eczema affected skin.17

Who should be careful, and when should you stop?

Do not use retinoids during pregnancy. Discuss plans for pregnancy or breastfeeding with your clinician before starting. Skin allergies, substantial dryness, redness or inflammation are reasons to seek advice first. Irritation can trigger dark marks in darker skin, making a gradual start and moisturizer particularly useful.46

Dryness, redness, scaling and burning are most common in the first two to four weeks and usually lessen. Contact your prescriber for stinging, burning or persistent irritation; reducing frequency or stopping can be necessary. Stop for a reaction suggesting allergy or significant irritation. Get immediate medical help for hives, breathing difficulty or swelling of the face, lips, tongue or throat.14
