Does sagging start in your 20s, 30s or 40s?

Those decades are not universal cutoffs. Your genes largely control the timing of natural changes such as thinner skin and loss of facial fullness. Environmental damage can make skin age prematurely, so age alone cannot tell you how much looseness to expect. A birthday is not a switch that makes skin loose, however convenient that would be for selling age brackets.1

Wrinkles are lines or creases; sagging is looseness or drooping. They can occur together, but noticing a fine line does not mean the skin has started visibly sagging. Wrinkles generally become more prominent after age 65, yet that figure describes wrinkles rather than a deadline for loose skin.3

Why does skin become less firm?

Collagen fibers support skin, while elastin fibers give it flexibility and strength. As connective tissue changes with age, skin loses strength and elasticity. The fat layer beneath the skin also thins, reducing padding and fullness. Sagging is therefore not just a change at the surface: the structures supporting the skin change too.4

Production of type I collagen, a major structural protein in skin, declines with aging. Existing collagen fibers also gradually fragment, leaving the supporting network more disorganized. Fibroblasts, the cells that make collagen, lose some of their attachment to that network and produce less new collagen. These are progressive changes rather than a sudden event at one particular age.5

Natural aging happens over time throughout the body. Environmental aging adds damage from factors such as ultraviolet radiation and lifestyle. Sun exposure is a major driver of skin aging, and ultraviolet light breaks down collagen fibers and disrupts elastin. Changes in elasticity are more noticeable on regularly exposed skin than on protected areas.234

What can you do before sagging becomes noticeable?

  • Protect exposed skin every day with broad spectrum, water resistant sunscreen with SPF 30 or higher. Seek shade and use protective clothing, a hat and sunglasses.
  • Avoid tanning beds and sunlamps. Their ultraviolet radiation accelerates skin aging, just as sunlight does.
  • Do not smoke. Smoking contributes to wrinkles and reduces production of new collagen.
  • Moisturize daily with a lotion or ointment. Aging skin becomes thinner, drier and less plump; moisturizing helps keep it comfortable.

Can skincare prevent or reverse sagging?

Preventive care can slow premature skin aging, but it cannot stop natural aging. Moisturizer traps water in the skin and improves its appearance; use it for hydration rather than treating it as a skin tightening procedure. Moisturizer and a facelift are different tools, despite the generous ambitions of the word "antiaging".1

Starting sun protection after visible changes appear still gives skin a chance to repair some sun damage. If looseness bothers you, a dermatologist can discuss skin tightening treatments. Facelift surgery removes excess skin and tightens tissue layers. Before choosing a procedure, discuss its risks and your current medicines, supplements and skincare.13

Stop ordinary skincare products that sting or burn. For a prescribed antiaging product, tell your dermatologist about irritation instead of assuming the same rule applies. Check your skin regularly, and have new or concerning lesions assessed rather than treating every skin change as aging.147
