Is it puffiness, an eye bag or a swollen eyelid?

Temporary puffiness happens when fluid collects in the thin tissue around your eyes. Crying can cause that buildup. Persistent bags can reflect inherited fat pad prominence, bone structure or loose skin, which gives an eye cream a fairly ambitious job description. Dark circles are different again: they are darkened skin below the eyes, and hollows called tear troughs can cast shadows that make puffiness look more noticeable.123

A swollen eyelid with a lump, crusting or itching needs a different approach from ordinary fluid puffiness. A stye is a small, red, painful bump caused by infection in an oil gland. A chalazion is a painless lump that can form after a stye. Blepharitis, inflammation of the eyelids, can cause redness, itching, burning and crusty eyelashes. Symptoms overlap, so an eye examination is more useful than choosing a treatment from appearance alone.4

How quickly should puffiness go down?

After crying, puffiness can fade within minutes or still be present the next morning. Time is usually the main remedy, with home treatments helping reduce swelling and irritation. A compress offers temporary relief rather than a permanent correction for recurring bags. Arrange an eye care assessment if unexplained swelling lasts longer than 24 to 48 hours, and seek help sooner for the warning signs below.12

Do eye creams or rollers help?

Eye creams containing caffeine or retinol can sometimes improve the appearance of bags. For swelling after crying, a chilled eye cream or serum you already tolerate is an option. Avoid testing a new product while the area is irritated: the skin around the eyes is thin and sensitive. These are options for improving appearance, not a treatment for an infected or painful eyelid.12

A face roller might help puffiness after crying. Roll lightly outward from the centre of your face and eye area, in one direction rather than back and forth. Gentle finger massage follows the same outward direction, starting near the nose. Stop if touching the area irritates it or makes swelling worse.1

What should you avoid putting around your eyes?

Avoid hemorrhoid cream around the eyes. Ingredients such as hydrocortisone or phenylephrine can cause irritation, skin changes or medication interactions. Witch hazel also lacks scientific support as a treatment for puffy eyes. If mascara, eyeshadow or eye cream seems to trigger swelling, stop using the suspected product for at least a couple of weeks to see whether symptoms improve.12

What if your eyelids are itchy or crusty?

Antihistamine eye drops help allergy related puffiness, not swelling from other causes. Use steroid eye drops only as prescribed. A pharmacist can advise on common eyelid problems and cleaning solutions for sticky eyelids, or direct you to an optician or doctor. Depending on the diagnosis, treatment can include warm compresses, medicines, ointments or eye drops. A cold compress is not interchangeable with every treatment for eyelid swelling.245

Rosacea can affect the eyes as well as facial skin. Signs include swollen red eyelids, bloodshot eyes, crusty lashes, watering or dryness, a gritty feeling, burning and sensitivity to light. Eye symptoms can appear before facial redness. If you think rosacea is affecting your eyes, arrange a dermatologist appointment right away: early treatment helps prevent eyesight problems and serious infection.6

What helps stop puffiness coming back?

Aim for seven to nine hours of sleep and raise your head slightly at night to reduce fluid buildup. Limiting salty foods can help because salt promotes water retention. Avoid rubbing or scratching your eyes, which can cause swelling and damage small blood vessels. For accompanying dark circles, use facial sunscreen and sunglasses, and reduce smoking and excessive alcohol use.23

What treats persistent bags or dark shadows?

Treatment needs to match the feature you want changed. Fillers add volume and smooth hollows, laser treatment can resurface and tighten skin, and chemical peels target pigmentation. Blepharoplasty, eyelid surgery, removes excess fat and skin. A bag, a hollow and darkened skin can share the same small address while needing different work done. Discuss these options with a healthcare provider rather than treating all three as puffiness.3

When should you get medical help?

Get urgent medical assessment if:

  • Your swollen eyelid is red, hot, painful, tender or blistered.
  • You cannot open your eye, or your eyelid suddenly droops.
  • You have pain inside the eye, marked redness, sensitivity to light or changes in vision.
  • Eye symptoms come with a headache, nausea, a high temperature or feeling generally unwell.
  • You suspect an allergic reaction.
5

A painful red eye, loss of vision, an eye injury or chemical exposure needs urgent medical attention. Do not press or rub an injured eye. If a chemical gets into your eye, flush immediately with a gentle stream of cool tap water for at least 15 minutes and seek medical help right away.7

Contact a healthcare provider for swelling under only one eye, excessive swelling or symptoms that are not improving. An eye care specialist can examine your eyelids and review your medical and family history to identify the cause. Keeping eyelids clean can help prevent irritation and infection, but persistent swelling needs more than another cosmetic product.34
