Why can a rich cream cause breakouts?
Acne develops when pores become plugged with skin oil and dead skin cells. Oily or greasy personal care products can increase the risk of those plugs. The resulting bumps include whiteheads and blackheads, together called comedones. Whiteheads are covered by skin; blackheads have an opening where the contents darken on contact with air. Occlusive moisturizers form a protective layer that reduces water loss. Ointments are generally more occlusive than creams, and creams more than lotions. That protection helps dry skin, but occlusive products can also cause or aggravate acne. A richer texture is therefore a reason to check the formula, not an automatic reason to reject every cream.13
What should you choose instead?
Look for an oil free, water based moisturizer labelled noncomedogenic, meaning unlikely to clog pores. "Won't clog pores" is another useful label phrase. For skin that is easily irritated, choose a formula without fragrance or essential oils: irritation can worsen existing acne. These descriptions help narrow the choice, but even a noncomedogenic product can cause breakouts for an individual. If a particular cream consistently worsens your acne, stop using that cream rather than treating its label as more reliable than your experience.2345
Should you stop moisturizing altogether?
Not if your skin is dry. Benzoyl peroxide, salicylic acid, retinoids such as adapalene or tretinoin, and prescription isotretinoin can dry and irritate skin. Daily moisturizer can help you tolerate these treatments. Moisturizer has a supporting role in an acne routine, even if the jar looks ready to run the entire department. Choose the moisturizer around the dryness you need to manage, rather than assuming oily skin never needs moisture.4
How should you apply it?
- 1Wash gently with a mild cleanser. Avoid scrubbing or repeatedly washing your face to remove oil.
- 2For moisturizing alone, leave a little water on your skin after washing, then apply moisturizer to help retain that water.
- 3Use moisturizer daily when your skin feels dry. Morning and evening moisturizing is an option for skin with blackheads and whiteheads.
- 4If an acne product leaves your skin dry and tight, moisturizer can be layered over it. Follow any specific medication instructions instead of assuming every treatment uses the same order.
For prescription retinoids, your dermatologist can recommend applying moisturizer first to reduce irritation. Changes to the amount of retinoid or how often you use it should be made with their help.6
How can you tell whether your moisturizer is the problem?
Remove the suspected product for two weeks and watch for fewer new breakouts. Improvement suggests you have found a possible contributor. Change one product at a time so you can identify the culprit, rather than giving the entire bathroom shelf an alibi. Before using a replacement across your face, apply it to a small area for a week. Watch for itching, burning or breakouts before expanding its use.5
How long should improvement take?
An acne treatment plan usually needs at least six to eight weeks before fewer breakouts appear. Blackheads and whiteheads can take eight to twelve weeks to start improving and several months to clear. Those are treatment timelines, not a reason to keep using a moisturizer that worsens your acne.36
See a doctor if acne is worsening, painful, leaving scars or causing emotional stress, or if skin care and pharmacy acne treatments have not helped after several months. Acne like bumps are not always acne; a dermatologist can check the diagnosis before you keep changing creams.26
