Why do people call it purging?

Azelaic acid helps reduce pore blockages, bacteria and inflammation. It improves how skin cells grow inside follicles, the openings where acne develops when oil and dead skin collect. That makes a temporary breakout sound plausible, but it does not give you a reliable way to label new spots as purging. Do not treat 'purging' as proof that azelaic acid is working, because skincare already has enough promotional vocabulary.345

Is it new acne or irritation?

Acne includes whiteheads, blackheads, raised spots and pus filled bumps. Irritation more often involves itching, burning, redness, dryness or scaling. Both can occur during treatment, so a breakout accompanied by uncomfortable, peeling skin should not simply be dismissed as a purge. Acne was reported in trials of prescription 15% azelaic acid gel for rosacea; that is different from an expected acne clearing phase.156

How should I use azelaic acid if my skin reacts?

General guidance for acne cream is to start once daily and increase if tolerated; prescription directions can differ. Use the directions for the product you actually have, rather than giving a cream the gel's paperwork.24

  1. 1Cleanse gently, then pat your skin dry. Both AZELEX 20% cream and prescription 15% gel direct application to dry skin.
  2. 2Apply the amount your product specifies. Those prescription products call for a thin layer in the morning and evening. Wash your hands afterwards.
  3. 3Keep irritating extras out of the routine. The 15% gel directions advise avoiding alcoholic cleansers, astringents, abrasives and peeling agents. Scrubbing can also make acne worse.
  4. 4Adjust persistent irritation rather than pushing through it. AZELEX directs reducing application to once daily or pausing until irritation subsides. If troublesome irritation persists, stop and contact your prescriber.

Why does azelaic acid make my face itch?

Itching is a known local irritation effect, particularly early in treatment. Azelaic acid's acidity partly explains irritation on broken or inflamed skin. Very sensitive skin and eczema can make application more irritating, even though azelaic acid is an option for sensitive skin and acne that leaves dark marks.1234

Why does azelaic acid sting when I apply it to damp skin?

Apply azelaic acid after patting your skin dry, rather than while it is still damp. The precise reason damp application stings more for you is not settled. Azelaic acid can cause burning on its own, and the formula and your skin's barrier affect how it enters the skin.16

Why does azelaic acid still sting after several weeks?

Early irritation is often mild and temporary, but there is no fixed deadline by which stinging must disappear. Persistent stinging needs a tolerance review, not a longer purge countdown. Stop and seek medical advice for severe redness, scaling, itching or burning. Stop treatment and seek urgent help for facial swelling, hives or difficulty breathing.147

How long should I wait for improvement?

Acne improvement usually starts around four weeks, with further improvement over several months and maximum results around six months of continuous cream use. Those are treatment timelines, not purge durations. See a doctor sooner if spots are deep, painful, causing scars or worsening despite treatment. If you are pregnant, discuss prescription use with your clinician; adequate controlled pregnancy studies are lacking.245
