Which kinds of heat rash can be less itchy?

TypeWhat it looks and feels like
Miliaria crystallinaTiny, clear blisters resembling water droplets. They break easily and can leave fine scaling. Itching or discomfort can be absent.
Miliaria rubraSmall, raised bumps that are usually very itchy. They often look red on lighter skin and can look grey or white on darker skin.
Miliaria profundaDeeper, skin coloured bumps. These can have no symptoms, although they can also be painful. This rare form follows repeated episodes of miliaria rubra.

What makes heat rash more likely?

Heat rash develops when the tiny tubes carrying sweat to the skin's surface become blocked and trap sweat inside the skin. Heat, humidity, exercise and clothing that traps sweat make it more likely. Heat rash can happen under too many layers even in cool weather, so the weather forecast does not get the final vote. Being bedridden can also contribute, especially where skin rests against bedding. Some medicines increase sweating; include your medication list if you have the rash assessed.14

Heat rash often appears beneath clothing or in skin folds with little airflow. In adults, common areas include the chest, back, arms and inner thighs. In babies, check the neck, armpits, elbow creases, diaper area, upper chest and back. A clinician considers the appearance, location and recent heat or sweating together rather than relying on itch alone. Irritation from soaps or household products can cause a rash that hurts more than it itches.134

What should you do if it does not itch?

Cooling and airing out affected skin are the main steps, which makes this a rare skincare problem with no shopping list.1

  1. 1Move to a cooler room, preferably with air conditioning or a fan, and pause activities that cause heavy sweating.
  2. 2Remove clothing covering the rash. Gently wash with cool or lukewarm water, then pat dry without scrubbing.
  3. 3Leave the area uncovered where practical, or wear clean, loose cotton clothing. Change out of wet clothing.
  4. 4Use lightweight bedding and keep your bedroom cool. Take cool baths or showers, and avoid perfumed shower gels.

Avoid thick ointments and products that block sweat from escaping. Anti itch treatments address discomfort rather than the trapped sweat itself. If discomfort develops, ask a pharmacist about calamine or another suitable treatment. Get a doctor's advice before using hydrocortisone cream on a child under 10. Most heat rash settles within a day or two after moving to a cooler environment, although clearing can take a few days.245

What if the rash is on a baby?

Keep your baby cool and dry, with lightweight, soft cotton clothing rather than extra layers. If using a fan, position it far enough away to give only a gentle breeze. Avoid baby powders, creams and ointments: powders do not prevent heat rash, and creams and ointments keep skin warmer and block pores. Contact your baby's doctor if you are worried, or if the rash repeatedly returns.16

When should you get medical help?

Arrange an assessment if the rash does not improve after a few days of cooling, becomes painful, or you suspect another cause. Seek medical advice for fever, chills, swelling, warm skin, pus or red streaks. Blisters around the eyes, mouth or genitals also need assessment. After heat exposure, dizziness, nausea, vomiting or headache can signal heat exhaustion and need immediate medical attention. Confusion or slurred speech can signal heat stroke, a medical emergency. Call emergency services for a sudden, rapidly spreading rash, trouble breathing or facial swelling.357
