Why can't seborrheic dermatitis be cured?

The exact cause is not fully understood. Seborrheic dermatitis appears to involve several factors, including oil gland activity, genes, changes in the skin's protective barrier and Malassezia, a yeast that normally lives on skin. Treatment reduces yeast, inflammation and scale, but adults often need ongoing maintenance because the condition can return. Clear skin between flares means the condition is controlled, not permanently gone. Seborrheic dermatitis is not contagious.134

How do you get it under control?

Treatment depends on where the rash is and how severe it is. Dandruff shampoos do different jobs, despite occupying the same rather unglamorous category. Some control yeast; others help remove scale.12

  1. 1Start with treatment for the affected area. For mild scalp symptoms, choose a medicated shampoo labelled for dandruff or seborrheic dermatitis. Follow its directions, including how long to leave it on before rinsing.
  2. 2Use the prescribed frequency. Dermatologists often suggest dandruff shampoo 2 to 3 times weekly for straight or wavy hair, but once weekly for curly or tightly coiled hair. Your treatment plan can differ.
  3. 3For the face or body, get the rash assessed. A common approach is a cleanser without soap once or twice daily, followed by an antifungal cream such as ketoconazole or ciclopirox. These creams are commonly used daily for 2 to 4 weeks, following the specific product or prescription directions.
  4. 4If inflammation persists, ask about additional treatment. A clinician can add a short course of a corticosteroid, a medicine that reduces inflammation, or consider alternatives such as pimecrolimus or tacrolimus.
  5. 5Once symptoms settle, move to maintenance rather than assuming treatment is finished. Your clinician can adjust the frequency to help prevent another flare.

Steroid creams manage inflammation; they do not cure seborrheic dermatitis. Mometasone is a stronger prescription steroid generally used when milder steroids have not worked. Use it only if prescribed for you, and only on your face if your doctor approves. Large areas or prolonged use also need your doctor's approval.5

How long does treatment take, and what if it returns?

Ketoconazole cream and shampoo usually take 2 to 4 weeks to work for seborrheic dermatitis. If symptoms are no better after 4 weeks, speak to a pharmacist or doctor; speak to your doctor sooner if symptoms worsen. When ketoconazole worked before, it can usually be used again for a recurrence. Some people continue ketoconazole shampoo every 1 to 2 weeks for months or years to prevent symptoms returning.6

If a shampoo stops helping, check its active ingredient and discuss switching or rotating treatments. A different bottle is not necessarily a different treatment, however dramatic the packaging change. Stress, cold and dry weather, and skincare containing alcohol can worsen symptoms, so noticing these patterns can help you manage flares.12

Does cradle cap need the same treatment?

No. Cradle cap usually resolves without medication. Baby shampoo and gentle removal of loosened scales can help. Dandruff shampoo is not recommended for children, so do not copy an adult scalp routine onto a baby.2

When should you see a doctor?

Get medical advice if self care or pharmacy treatments are not helping, or patches drain fluid or pus, develop crusts, or become very red or painful. Seek help if symptoms affect daily life or mental health. Ketoconazole can cause burning, stinging or irritation; tell your clinician about troublesome reactions. If prescription ciclopirox shampoo irritates your scalp, stop using it and tell your dermatologist.123
