What do sweat rash blisters look like?

Miliaria crystallina, the surface form of heat rash, causes clear blisters about 1 to 2 mm across. They resemble beads of sweat, break easily and can leave fine flakes. There is no surrounding inflammation. These blisters commonly appear on the head, neck and upper body. Miliaria rubra, the more common form, causes very itchy red bumps and small blisterlike bumps about 2 to 4 mm across. In adults, it often affects the upper body, scalp, neck and folds where clothing rubs.1

Is every rash after sweating a sweat rash?

No. Sweat pimples and heat rash can look similar, but they are different conditions. Heat rash traps sweat in blocked sweat ducts and can feel prickly, itchy or burning. Sweat pimples develop through a combination of oil, bacteria, heat and friction that clogs pores and causes inflammation. Sweating alone does not explain every bump after exercise.2

Other rashes can also blister or leave raw skin. Irritants and allergies are possible causes, and contact dermatitis develops where skin touches an irritating substance or something it is allergic to. The cause matters when choosing treatment: a rash appearing after a hot day is not enough to identify it.3

What should you do first?

  1. 1Move into a cooler, ventilated space to reduce sweating.
  2. 2Remove wet clothing and extra layers. Change into loose, breathable clothing.
  3. 3Apply a cool, damp washcloth or take a cool bath. For children, a lukewarm bath or shower is another option.
  4. 4Dry away excess moisture. Avoid excessive soap and other skin irritants.

Cooling is the main treatment; calamine lotion can soothe discomfort. Cooling takes priority over adding products, which is an unusually short shopping list for skincare.5

What if the blisters are on a baby?

Babies can develop little red bumps and tiny blisters when their sweat glands become blocked, particularly in hot or humid weather. Keep your baby cool and dry, and use lightweight, soft cotton clothing. If using a fan, position it far enough away to give only a gentle breeze. Avoid powders, creams and ointments for baby heat rash: powders do not prevent or improve it, while creams and ointments can keep skin warmer and block pores.6

For children, avoid heavy fabrics, overdressing and too many blankets during sleep. Breathable cotton allows air to circulate and sweat to evaporate, giving clothing a more useful assignment than simply matching.4

How long should it take to improve?

Most heat rash clears within a day or two after moving to a cooler environment. In children, it typically clears within a few days without permanent skin damage. Heat rash is not contagious.14

When do blisters need medical attention?

Tiny clear blisters can be part of heat rash, but do not dismiss worsening blistering or scabs in a child. Call their doctor if the rash worsens, fails to improve within a few days, or comes with fever, chills, pain, swelling, warmth or swollen glands. A child with uncomplicated heat rash should otherwise act normally, without fever or signs of illness. Adults should get a persistent or severe rash assessed too.34

Get emergency medical help for a rapidly spreading rash after starting a new medicine or having an infection, especially with painful blistering or peeling. Sores involving the mouth or eyes, trouble breathing, or swelling of the mouth, tongue or throat also need immediate help. These can signal a serious reaction rather than ordinary heat rash.7
