Do you need to stop after a few months?

Clearing acne is not an automatic stopping point. Differin tells users to continue its adapalene 0.1% gel after skin looks clearer to prevent future breakouts. Continuing acne maintenance helps prevent new breakouts, so a clearer face is not a cancellation notice for the routine that helped clear it.23

For prescription Differin 0.3% gel, a one year safety trial involving 551 people found a similar pattern of side effects to the 12 week trial. Irritation was generally most likely early in treatment and usually lessened with continued use. Long term use still needs adjustment if side effects become troublesome; persistent irritation is not something to accept indefinitely.4

Adapalene is not an antibiotic and does not follow the same short course approach. Oral antibiotics for acne are usually prescribed for three to four months, or longer when needed. After clearing, a maintenance treatment applied to the skin can help keep acne controlled and reduce the need for another antibiotic course. If your prescription combines treatments, follow the maintenance plan rather than assuming every medicine should continue.13

How should you use adapalene over time?

  1. 1Wash gently with a mild cleanser and pat dry. Avoid scrubbing or abrasive cleansers.
  2. 2Apply a thin layer over the whole affected area, not just individual pimples. Keep adapalene away from eyes, lips and the inside of the nose.
  3. 3Use it once daily as directed. Adapalene is usually applied at bedtime; Differin 0.1% gel allows morning or evening use, while prescription Differin 0.3% gel specifies evening application.
  4. 4Follow with moisturizer. If applying Differin 0.1% gel in the morning, let the gel dry before sunscreen.
  5. 5Keep using sunscreen as labeled, limit sun exposure and avoid tanning beds. Do not double the application to compensate for a missed dose.

The useful supporting cast is cleanser, moisturizer and sunscreen, not a bathroom shelf auditioning for a department store. Differin advises avoiding salicylic acid, glycolic acid and other alpha hydroxy acid products because they can worsen irritation. Tell your prescriber about other acne medicines and skincare before combining them. Avoid waxing treated areas.124

What should you expect, and when should you get help?

Acne can initially worsen, and the full benefit can take 8 to 12 weeks or longer. Dryness, redness, peeling and burning or stinging are especially common during the first two to four weeks. Applying more will not speed up results. If there is no improvement after about 12 weeks, arrange a treatment review rather than continuing unchanged for months.124

For sensitive skin, discuss irritation with your doctor; moisturizer or less frequent application can help. Stop and seek medical advice if irritation becomes severe or persists. Do not apply adapalene to sunburned, broken or eczema affected skin until it has healed. Stop using it and seek urgent medical help for hives, facial or throat swelling, or difficulty breathing.124

See a doctor sooner if acne is deep, painful, causing scars or affecting your mental health. Those are reasons to reassess treatment, not simply extend the same routine.5

What if you become pregnant or breastfeed?

Discuss adapalene with your doctor if you are pregnant, planning pregnancy or breastfeeding, and call your doctor if you become pregnant during treatment. Pregnancy safety is uncertain. During breastfeeding, use the smallest amount and never apply it directly to the nipple or areola.14
