Stinging alone does not separate them: chapping can sting, and cold sores can burn before anything is visible. Watch for the change into blisters, then oozing and crusting. A cold sore can also crack or bleed as it heals. If the appearance is unclear, a clinician can examine the area and sometimes test fluid from a sore.123

What should you do for chapped lips?

Lip balm helps seal in existing moisture, a less glamorous job than the word "healing" on the tube suggests.1

  1. 1Choose a fragrance free ointment based balm containing petrolatum, glycerin or mineral oil. Avoid camphor, eucalyptus and menthol, which can dry or irritate lips.
  2. 2Apply balm in the morning, at bedtime and before exposure to the elements.
  3. 3Use an SPF 30 or higher lip balm outdoors. Reapply about every two hours during a long day outside.
  4. 4Leave peeling skin alone. Licking dries lips further, and picking or biting can create cracks and sores.

What should you do for a cold sore?

Contact your clinician at the first familiar tingling or itching rather than waiting for a blister. Antivirals work best when started early, particularly within the first 48 hours of a cold sore forming. Prescription tablets can reduce pain and speed recovery. Antiviral creams are another option, but often shorten an outbreak by only a few hours to a day.34

For discomfort, hold ice wrapped in a washcloth against the sore. Avoid hot drinks, spicy or salty foods and citrus if they hurt. Numbing creams offer temporary pain relief but do not shorten the outbreak. Frequent or severe outbreaks are a reason to discuss daily prescription antivirals with your clinician.234

Avoid kissing and oral sex during an outbreak, and do not share lip balm, cups or utensils. HSV spreads most easily with an active sore, but transmission is possible without visible blisters. The virus remains in the body after the sore heals; treating an outbreak does not remove it.345

Can cold weather cause chapped lips?

Yes. Cold, dry air and wind can cause chapping, and low humidity indoors increases moisture loss too. A humidifier can help. A scarf protects lips from cold and wind, giving an ordinary piece of fabric a second job in skincare. Cold weather and cracked skin can also trigger cold sores in someone who already carries HSV; the weather does not create the virus.1567

How long should healing take, and when should you get help?

Chapped lips usually improve quickly with balm, though severe cracking and bleeding can take two to three weeks to recover. Cold sores generally resolve in one to two weeks. Get chapping checked if home care brings no relief, or a sore checked if it lasts longer than two weeks.124

Seek medical care promptly for sores near an eye, severe symptoms, or suspected herpes with a weakened immune system, including from medicines. Fever, a rash or difficulty swallowing also warrant assessment. Persistent chapping can reflect an allergy or another health problem rather than weather alone.124
