Is it cystic acne or an ordinary pimple?

An acne cyst is a painful lump containing pus deep beneath the skin. A nodule is also deep and painful, but feels more solid because it does not contain fluid. Both can scar. Cysts can occur on the face, neck, chest, back, shoulders and upper arms, not just along the jawline. A dermatologist can examine the bumps and confirm what they are.3

Nodulocystic acne means severe acne with deep nodules and cysts. The lumps can be firm, tender or softer to the touch, and several can occur together. Acne involves oil production, blocked hair follicles, microorganisms and inflammation. Family history, certain medicines, anabolic steroids and whey supplements can contribute to nodulocystic acne. These are reasons to discuss your health and supplements during an appointment, not reasons to diagnose yourself.2

What should you do at home?

  1. 1Wash gently after waking and before bed. Use a mild foaming cleanser, lukewarm water and your fingers rather than a washcloth or sponge. Cleanse after exercise or sweating too.
  2. 2Remove makeup before bed. Choose makeup and facial products labelled noncomedogenic, meaning formulated not to clog pores.
  3. 3Apply an oil free moisturizer if your skin feels dry.
  4. 4For a painful area, wrap an ice pack in a washcloth or thick paper towel and apply gently for about 10 minutes. Never put ice directly on the skin. Stop immediately if pain increases or the area starts to feel numb.
  5. 5Leave cysts and scabs alone. Do not squeeze, pierce or attempt to drain them.

For acne on the chest or back, wear loose, breathable clothing, especially during exercise or sweating. Avoid scrubbing: it irritates skin and can make acne worse. Gentle washing and removing makeup are useful daily habits, which is a disappointingly short shopping list for an industry with so many shelves.5

Can benzoyl peroxide or salicylic acid clear cysts?

Benzoyl peroxide and salicylic acid are options for acne treatment applied to the skin, and a pharmacist can help you choose. Their place in a routine depends on which spots you have. A mixture of blackheads, whiteheads, surface pimples and deep lumps needs a different plan from surface acne alone.5

Topical treatments, meaning medicines applied to the skin, are usually ineffective for the deep nodules of nodulocystic acne. A dermatologist can still prescribe benzoyl peroxide or a topical retinoid for the surface spots alongside treatment for the deeper acne. An acne wash can have a supporting role, but its job title is not 'entire treatment plan'.6

For blocked pores and surface acne, benzoyl peroxide strengths of 2.5% to 10% are used. Medicines for blocked pores are generally applied as a thin layer over the affected area rather than only individual spots. Those directions are general acne guidance, not a cyst treatment or a reason to add products to an existing prescription routine. If you take isotretinoin, your dermatologist can recommend a very mild cleanser instead of an acne wash.46

Which home remedies should you skip?

  • Skip crushed aspirin masks. The concentration can be uneven across the skin; aspirin is not interchangeable with a formulated salicylic acid treatment.
  • Avoid vinegar as a cystic acne treatment. It can be drying or irritating.
  • Do not rely on tea tree oil for cystic acne. Evidence of benefit for mild acne does not make it a treatment for deep cysts, and it can cause an allergic skin reaction.
  • Leave extraction to a professional. Popping or draining a cyst at home increases scarring, inflammation and infection risk.

What treatment can a doctor offer?

Prescription treatment can combine medicines taken by mouth with medicines applied to the skin. Oral antibiotics help control bacteria involved in inflammation and pus. Topical retinoids such as adapalene or tretinoin reduce inflammation and oiliness and help prevent spots. Hormonal treatment, such as the combined contraceptive pill, is another option when hormones contribute to acne.5

For severe nodulocystic acne, oral isotretinoin is a recommended treatment, started early to prevent scarring. Treatment lasts at least five months, and some people need another course. When isotretinoin is unsuitable or unavailable, a dermatologist can consider systemic antibiotics or hormonal treatment. These are medical treatment choices, not medicines to start on your own.26

A dermatologist can use a corticosteroid injection to shrink a large, painful cyst quickly. Professional incision and drainage is another option for selected cysts. For women, spironolactone or a birth control pill can help lower hormone effects that contribute to cystic acne. Ask which options suit your acne and what side effects to expect.3

How long does cystic acne take to clear?

Clearing acne cysts can take three months or more with treatment. Flat light or dark marks left after a cyst clears can take more than a year to fade; these colour changes are different from scars. In skin of colour, pigmentation changes and raised keloid scars are common complications of nodulocystic acne. Severe acne can persist into adulthood, so waiting to outgrow it is not a reliable treatment strategy.23

Will changing your diet help?

Diet is not an established cure for cystic acne. Evidence linking specific foods to acne is weak, although high dairy intake affects some people. Skim milk and whey protein are worth discussing if you consume a lot of either. If you notice repeated flares after a particular food, discuss that pattern with your clinician rather than replacing treatment with a restrictive diet.46

When should you get medical help?

See a GP or dermatologist for deep, painful lumps, scarring, numerous spots or acne that is not improving with self treatment. Get medical advice for hot, swollen, red skin suggesting infection; redness can be harder to see on brown or black skin. Acne affecting your mental health is also a reason to seek treatment.5

Seek urgent medical assessment if severe acne starts abruptly with ulcerated lumps, fever, painful joints or feeling generally unwell. This combination can occur with a rare severe form called acne fulminans and needs specialist care.6

Before your appointment, list the treatments you have tried. Explain specifically how acne affects work, sleep or social plans, and say what you want treatment to achieve. Ask when to expect improvement, which side effects to watch for and how to prevent scars. Those details help shape a treatment plan you can follow.7
