Where can the rash spread?
The rash does not always start around the mouth. It can begin in the folds beside the nose and spread around the mouth, eyes or forehead. More severe cases can extend to the chin, cheeks and eyelids. A narrow strip of skin immediately beside the lips often stays clear.34
Occasionally, the rash affects the genitals. Spread to the ears, neck, scalp, trunk or limbs is much rarer. When several areas around body openings are involved, the condition is also called periorificial dermatitis. A rash in a new location needs assessment rather than an assumption that every new bump belongs to the same condition.125
Is it spreading dermatitis or acne?
Perioral dermatitis often looks like clusters of small pimples with dry, flaky surrounding skin. The bumps can be red on lighter skin or skin colored on darker skin. Itching and burning are possible, but neither is required. Unlike acne, perioral dermatitis does not typically produce blackheads, whiteheads, deep nodules or cysts. A dermatologist can distinguish it from acne and other facial rashes before you choose treatment.256
What can make it more extensive?
The exact cause is unknown. Topical steroids, including hydrocortisone, can temporarily improve the appearance while eventually making the rash more extensive and inflamed. Thick moisturizers, oily cosmetics and other irritating products can also aggravate it. Continuing a personal trigger can worsen the condition, but a spreading rash does not identify the trigger by itself.136
What should you do if it is spreading?
- 1Arrange a medical assessment if the rash is expanding, persistent or not clearly diagnosed. Perioral dermatitis is usually diagnosed by examining the skin; tests can help rule out infection or another type of dermatitis.
- 2Review steroid use. Stop hydrocortisone applied without your doctor's knowledge, but check with the prescriber before changing a prescription steroid. Strong facial steroids sometimes need a supervised gradual withdrawal.
- 3Reduce facial cosmetics and extra skincare products. Pause facial cosmetics during treatment, giving the bathroom shelf a rare opportunity to do less.
- 4Wash gently. Warm water alone is one option during a flare; a mild, fragrance free cleanser is another. Avoid heavy, occlusive creams and ointments. A bland moisturizer can help dryness, and a gel or liquid sunscreen is an option.
If a cleanser is part of your treatment routine, gentle cleansing twice daily is an option. A mild cleanser is enough for washing, without turning the sink into a product demonstration.6
The rash can flare after stopping a topical steroid. Do not restart it for temporary relief unless your clinician directs you to. Continue medically necessary inhaled, nasal or oral steroids; rinsing the face and mouth after nasal or inhaled steroids can help.14
How long does it take to settle?
Expect gradual improvement over weeks to months, not immediate clearing. Without treatment, the rash can persist for months or years. Some people improve by removing triggers; others need prescription creams, gels or oral antibiotics that help reduce inflammation. The rash can return after successful treatment.125
Tell your clinician if you are pregnant or the patient is a child, because antibiotic choices differ. If eye redness develops alongside the rash, seek assessment; an eye specialist referral is sometimes needed. Persistent spreading despite routine changes is also a reason to see a dermatologist.124
