Does seborrheic dermatitis ever go away?

Yes, the visible rash, flakes and itch can settle. A clear scalp is a treatment goal, but keeping it clear often means continuing a maintenance routine rather than stopping everything as soon as the flakes disappear.2

Seborrheic dermatitis affects oily areas such as the scalp. It is associated with Malassezia, a yeast normally present on skin. Higher amounts of this yeast and the irritating substances it produces contribute to scalp sensitivity and flaking. Stress and cold weather can also be associated with symptoms returning.34

How do you get it under control?

  1. 1Use a dandruff shampoo as directed. Mild to moderate scalp seborrheic dermatitis sometimes needs no other treatment.
  2. 2Match washing frequency to your treatment plan and hair type. Straight or wavy hair often allows treatment 2 to 3 times weekly; curly or tightly coiled hair may need treatment once weekly.
  3. 3Leave the shampoo on your scalp for the directed contact time. Rinsing immediately does not give the medication its intended time to work.
  4. 4Once symptoms settle, follow the maintenance plan. Dandruff shampoo once weekly is often used to prevent further flares.

Different shampoos do different jobs. Ketoconazole targets yeast, while salicylic acid helps lift and reduce scale. Ketoconazole 2% shampoo is used twice weekly for seborrheic dermatitis, but follow the directions for your particular product. In the UK, this strength can be bought over the counter. If a previously helpful shampoo stops working, discuss rotating shampoos with your dermatologist.2356

How long does treatment take?

Expect a course of treatment rather than one wash. Prescription ciclopirox shampoo is commonly used for 4 weeks. If it causes scalp irritation, stop using it and tell your dermatologist; also report no improvement after 4 weeks. If shampoo or antifungal treatment is insufficient, a dermatologist can add a corticosteroid to reduce inflammation and itch quickly.2

Which hair care changes help?

A shampoo can clean hair without much lather, so the bubbles are not its performance review. For an easily irritated scalp, avoid cosmetic shampoos containing sodium lauryl sulphate and fragranced products, including essential oils. A fragrance free, moisturising shampoo can help with accompanying dryness.3

Conditioner is designed to keep hair smooth and shiny, a job description that does not include treating seborrheic dermatitis. Conditioners and styling products can irritate scalp eczema. Reduce products that cause irritation, then reintroduce styling products individually to identify a problem. Keep the hairdryer on a low or warm setting rather than hot.3

Does seborrheic dermatitis go away with age?

For adults, plan around controlling symptoms and preventing their return rather than waiting to outgrow them. Cradle cap usually resolves within weeks to months; baby shampoo and gentle removal of loosened scales can help. Dandruff shampoo is not recommended for children. Atopic dermatitis is a different form of eczema that many children outgrow, so its age pattern should not be applied to adult seborrheic dermatitis.127

When should you see a doctor?

Get reviewed if treatment works poorly, symptoms worsen or there are signs of infection. Once the diagnosis is established, many people can manage without routine appointments. Patchy hair loss, broken hairs or an oozing scalp deserve assessment rather than another shampoo experiment: hair loss is uncommon in seborrheic dermatitis, and other scalp conditions can cause similar flakes and itch.146
