What clues separate heat rash from acne?
| Clue | Heat rash | Acne after sweating |
|---|---|---|
| What gets blocked | Sweat ducts, trapping sweat in the skin. | Pores, when sweat mixes with bacteria and oils. |
| Useful clues | Small raised spots with itching, prickling or mild swelling. Some contain fluid. | A humid, sweaty day can cause breakouts if your skin is already acne prone. |
| First practical step | Cool the skin and reduce sweating. | Blot sweat gently and avoid products that clog pores. |
The common itchy form of heat rash often affects the upper body, neck, scalp and skin folds, especially where clothing rubs. The bumps are not centred on hair follicles. Heat rash can look red on white skin, but grey or white on brown or black skin, where it can be harder to see. Trapped sweat can cause a rash a few days later, so timing alone cannot settle the question.13
What should I do now?
- 1Move somewhere cooler. Use a fan or air conditioning, take a cool shower or bath, and drink fluids to avoid dehydration.
- 2Remove wet clothing and change into loose cotton clothing. Keep your bedroom cool and use lightweight bedding.
- 3Calm prickling with a cool damp cloth, or an ice pack wrapped in a tea towel, for up to 20 minutes. Pat rather than scratch the rash.
- 4Avoid perfumed shower gels and creams, excessive soap and other irritants.
- 5If breakouts are the concern, blot sweat with a clean towel rather than rubbing. Wash sweaty clothes, headbands, hats and towels before using them again.
Look for products labelled "noncomedogenic", "oil free" or "won't clog pores", which is more useful here than a tropical name on the bottle. For recurring heat rash, exercise outdoors during the coolest part of the day or move your workout into air conditioning.2
How long should heat rash last?
Most heat rash clears within a day or two after moving to a cooler environment; some cases take a few days. A pharmacist can advise on calamine lotion, antihistamines or hydrocortisone cream for discomfort. Children under 10 need a doctor's advice before using hydrocortisone. Calamine can dry the skin.13
See a doctor if the rash has not improved after a few days or is worsening. Itchy, tender bumps around hair follicles can be folliculitis, an infection of those follicles, rather than acne. After a workout, changing out of tight clothing and showering helps reduce that risk.12
How can I manage rosacea when hot weather triggers flushing?
Rosacea can cause flushing and bumps that resemble acne, usually around the nose and cheeks. Hot weather is a recognised trigger. Track your triggers, wash with a cleanser made for sensitive skin, moisturise regularly, and avoid exfoliants and products containing alcohol. Use facial sunscreen with SPF 30 or higher every day. Treatment often combines skincare and medicines, with improvement taking three months or longer. If you develop eye irritation, see a healthcare provider right away.4
