How to check the area

  1. 1Start with an area where the acne has healed. A painful, pus filled or deep pimple still needs attention as active acne rather than scar treatment.
  2. 2Gently run a finger over the area. A raised scar has height you can feel; a pit or indentation suggests a depressed scar instead.
  3. 3Look at the outline. A hypertrophic scar, meaning a raised scar contained within the original damaged area, stays inside that boundary. A keloid grows beyond it.

Flat does not automatically mean pigmentation: some scars flatten as they heal. A dermatologist checks size, texture, colour and the history of the area to identify the type. The word 'mark' describes appearance, not a diagnosis, which is a lot of responsibility for four letters.2

Will a raised scar flatten by itself?

Hypertrophic scars can get smaller over time but do not completely flatten. Keloids can grow beyond the original injured area. Scars usually fade, but scar tissue does not disappear completely. Treatments can make scars less noticeable, even if 'removal' would look tidier on a menu.24

Why the difference changes treatment

Persistent brown marks after acne are called postinflammatory pigmentation, meaning extra colour left after inflammation. Azelaic acid can reduce this pigmentation and treat mild to moderate acne. That makes it an option for brown marks, particularly in darker skin tones, rather than a reason to treat every raised area with a fading cream.1

For azelaic acid cream, begin with daily application to acne affected skin after cleansing, then increase to twice daily if tolerated. Acne improvement often begins around four weeks, with maximum results after six months; that is an acne timeline, not a deadline for brown marks to disappear. Very sensitive skin or eczema can make application irritating. Stop and seek medical advice for severe redness, scaling, itching or burning.1

Raised acne scars have different treatment options. A dermatologist can use steroid injections to flatten them, usually with multiple treatments before a noticeable difference. Silicone ointment or silicone gel sheets are other scar options to discuss. If acne is still active, getting breakouts managed comes first: fewer breakouts mean fewer new scars. Treatment should match the scar's type, size, location and symptoms, not just its colour.25

Do not use a home microneedling roller to test whether a bump will improve. Home devices can cause infection, scarring or colour changes. Microneedling stimulates collagen, the protein that forms much of scar tissue, and is not suitable if you have or have had a keloid. A dermatologist should decide whether the procedure fits your scar.23

When should I get the bump checked?

Arrange an examination if the raised area is growing, painful or affecting movement, or if you cannot tell whether acne has healed. Mention itching or tenderness too. Bumps or raised scars on the back of the neck or scalp deserve particular attention: acne keloidalis nuchae can resemble acne or razor bumps but is a different condition. Without medical treatment, it can worsen and cause raised scarring and permanent hair loss; home care alone cannot treat it effectively.26
