Why does acne appear on your back?
Acne develops when hair follicles become blocked with oil and dead skin cells. Bacteria can grow inside blocked follicles and cause inflammation and pus. Your upper back has many oil glands, making it a common place for breakouts. Hormonal changes, certain medicines, greasy skin or hair products, and tight clothing can contribute. Treatment needs to prevent new blockages as well as deal with existing spots; washing alone does not address every cause.34
Which spots can you treat at home?
A few blackheads, whiteheads, or small pimples are a reasonable starting point for pharmacy treatment. Blackheads and whiteheads are blocked pores; pustules are raised spots containing pus. Inflamed bumps can look pink or red on white skin and brown or purple on brown and black skin. Nodules and cysts are larger lumps beneath the skin and can be painful. These deeper spots carry a greater risk of scarring, so arrange medical treatment early rather than waiting for them to burst.25
Which acne treatment should you choose?
| Treatment | What it does | Where it fits |
|---|---|---|
| Benzoyl peroxide wash | Reduces bacteria that contribute to acne. | A practical daily treatment for mild back acne. |
| Adapalene 0.1% gel | Unclogs pores and helps prevent new pimples. | Can be used alongside benzoyl peroxide; usually applied at bedtime. |
| Salicylic acid | Loosens dead skin cells and opens clogged pores. | An option in cleansers or lotions for blocked pores and pimples. |
| Azelaic acid | Opens clogged pores, reduces inflammation, and works on acne bacteria. | An alternative when benzoyl peroxide or a topical retinoid is too irritating. |
Lower strength benzoyl peroxide and wash formulas are less irritating than stronger options. A 5.3% foaming wash is less likely to cause dryness, peeling, or irritation than a stronger wash; 10% is the strongest nonprescription concentration. Stronger is not automatically the better starting point, particularly if your skin is sensitive.14
How to use a back acne routine
- 1Wash gently with lukewarm water. Keep routine washing to no more than twice daily, using a mild cleanser rather than scrubbing.
- 2Use your benzoyl peroxide wash daily. Leave it on your back for two to five minutes before rinsing thoroughly. Benzoyl peroxide can bleach clothes, towels, and bedding.
- 3If using adapalene, gently cleanse and pat the skin dry first. Spread a thin film over the entire affected area, not just individual spots, usually once daily at bedtime. Follow the package or prescription directions.
- 4If your skin becomes dry, use a fragrance free, water based moisturizer. Choose products labelled noncomedogenic, meaning less likely to block pores.
- 5Protect exposed skin outdoors with noncomedogenic, water resistant, broad spectrum sunscreen with SPF 30 or higher. Broad spectrum protection covers UVA and UVB rays.
A lotion applicator can help you reach your back, a rare skincare accessory that solves a problem of geography.1
What should you change after exercise?
Wear loose cotton or sweat wicking workout clothes, wash them after each use, and shower and change promptly after sweating. If a shower is not possible, gently wipe your back with an oil free cleansing wipe and change out of damp clothes. A handheld bag avoids backpack friction, so your luggage can stop participating in your skincare routine.1
What makes back acne worse?
Do not pick or squeeze spots: both can worsen acne and cause permanent scars. Scrubbing also irritates acne rather than clearing it. Avoid very hot or cold washing water and heavy, greasy products. Toothpaste is not an acne treatment; its ingredients can irritate and damage skin. If you already use prescription acne medicines, tell your doctor or pharmacist before adding another treatment so the products can be considered together.357
Who should be careful with adapalene?
Adapalene treats acne in adults and children aged 12 and older. Do not apply it to broken, sunburned, or eczema affected skin until the skin has healed. Discuss pregnancy, plans to become pregnant, or breastfeeding with your doctor before use; call your doctor if you become pregnant during treatment. Avoid waxing the treated area and unnecessary sun exposure. Applying extra medication does not improve results and can increase irritation.7
Dryness, redness, scaling, or stinging can occur during the first two to four weeks. Contact your doctor if irritation is severe, persists, or your skin burns. Your doctor can advise on moisturizer or less frequent application. Stop using adapalene and get urgent medical help for hives, difficulty breathing, or swelling of the eyes, face, lips, tongue, or throat.7
How long does back acne take to improve?
Back acne treatment can start showing results in six to eight weeks, with complete clearing taking three to four months. Adapalene can bring previously unseen pimples to the surface during the first few weeks, and its full benefit can take eight to twelve weeks or longer. Early worsening is different from severe irritation: do not push through burning or painful skin. If there is no improvement after six to eight weeks, arrange a medical review.17
When should you see a doctor?
- Seek help for numerous spots, deep painful lumps, or developing scars.
- Get medical advice for hot, swollen, red skin that could indicate infection. Redness can be harder to see on brown and black skin.
- Ask for help if acne is affecting your mental health, even if the spots look mild.
- Have sudden, severe adult acne assessed. New acne alongside irregular periods or excessive body hair also warrants a medical discussion.
- If the diagnosis is unclear, have the bumps examined. Folliculitis, an inflammation or infection of hair follicles, can look like acne and needs a different treatment plan.
What can prescription treatment do?
A doctor can select stronger topical treatments or tablets based on the type and severity of your acne. Topical antibiotics are used with benzoyl peroxide to reduce antibiotic resistance, which means bacteria becoming less responsive to treatment. Oral antibiotics can treat moderate to severe inflamed pimples and are generally used for the shortest necessary course, usually several months, alongside another acne medicine. Pregnancy and breastfeeding change antibiotic choices, so tell the prescriber.256
For severe acne, a specialist can consider isotretinoin, an oral retinoid that shrinks oil glands. It requires medical supervision because it can cause a wide range of side effects. Hormonal treatments, including certain combined contraceptive pills, are another option for some women. The choice depends on your medical circumstances, not simply on how many products you have already tried.345
What about marks left after spots clear?
Dark marks after acne and scars that change the skin's texture are different problems. Retinoids and azelaic acid can help treat dark spots after acne clears. Retinoids can also be used as maintenance treatment once breakouts are controlled. Preventing new spots and avoiding picking help limit further scarring; existing indentations or other texture changes deserve a separate discussion with a dermatologist.26
