What counts as cystic acne?

Cystic acne causes deep, pus filled lumps rather than just small surface pimples. Nodules are deeper lumps that can feel firm and tender; cysts and nodules often occur together, sometimes called nodulocystic acne. Breakouts can affect the face, neck, chest and back. Blackheads and whiteheads can appear alongside them. Hormonal acne describes acne linked to hormone changes, while cystic acne describes the deep breakouts themselves: the two can overlap.2345

When should you see a doctor?

Book an appointment if spots are deep, painful, numerous or leaving scars. You do not have to exhaust every pharmacy treatment first. A GP can prescribe treatment or refer you to a dermatologist. Seek medical help for signs of infection, including hot, swollen and red skin; redness can be harder to see on brown and black skin. Tell the doctor if acne affects your mood, confidence, work or social life. Anxiety, depression and avoiding other people are reasons to seek help, not reasons to wait until the acne looks worse.46

How to care for your skin each day

  1. 1Wash gently in the morning and at night. Apply cleanser to wet skin with your fingers and massage without scrubbing.
  2. 2Rinse with warm water, then pat dry with a soft, clean towel. Washing more than twice daily can strip skin of oils.
  3. 3Use your prescribed skin treatment as directed. Acne medicines applied to the skin come as creams, gels or lotions; the schedule depends on the medicine.
  4. 4Remove makeup at the end of the day. Choose skincare and makeup labelled noncomedogenic, meaning formulated not to clog pores.
16

For sensitive or easily dried out skin, a creamy cleanser can provide more hydration than a foaming cleanser. Avoid harsh exfoliating washes and scrubbing, which can irritate skin and worsen acne. Do not pick, squeeze or try to drain a cyst. Picking can cause scars, and draining a deep lump at home carries a substantial infection risk. For chest or back acne, wear loose, breathable clothing, especially during exercise or sweating.146

Can an acne wash clear cysts?

An acne wash can support a routine, but severe nodulocystic acne usually does not respond to topical treatment alone. Benzoyl peroxide fights bacteria, clogged pores and inflammation; washes are available in strengths from 2.5% to 10%. Salicylic acid washes commonly contain 2%. These are options to discuss with a pharmacist or doctor, not a reason to postpone medical care. If a wash or added skincare product leaves your skin dry and irritated, review the product with them rather than adding more exfoliation.12

Which medical treatments help?

TreatmentRole in managing acne
Oral antibiotic plus skin treatmentA common starting approach for severe acne. The antibiotic reduces inflammation, while skin treatment addresses bacteria and blocked pores.
Topical retinoidsMedicines such as adapalene and tretinoin reduce inflammation and oiliness and help prevent spots.
IsotretinoinAn oral medicine for severe acne that addresses excess oil, blocked pores, bacteria and inflammation.
Hormonal treatmentCertain birth control pills and spironolactone can help suitable patients with hormone related acne.
Corticosteroid injectionA dermatologist can treat an individual painful cyst or nodule to reduce its size and pain quickly.

For nodulocystic acne, early oral isotretinoin treatment can help prevent scars. Treatment is generally needed for at least five months, and some people need another course. Isotretinoin has possible side effects and requires a monitoring program. Discuss the benefits, risks and monitoring before deciding. A clinician can also drain a large, painful cyst that medicine has not cleared; this is a medical procedure, not something to attempt at home.24

What if hormones are contributing?

Hormone changes can increase sebum, the oil made by skin glands, and contribute to blocked pores. Tell your clinician about changes around periods, pregnancy, menopause or stopping birth control. Persistent lower face acne can occur with an ovarian hormonal condition, but the location or appearance of breakouts cannot diagnose one. Hormone tests or other investigations are sometimes needed. Certain birth control pills reduce hormones that trigger acne, but not every pill has that effect. Spironolactone can help hormonal acne when other treatments have not worked. The choice depends on your medical history and pregnancy plans.57

What changes during pregnancy?

If you are pregnant or planning pregnancy, discuss acne treatment with your healthcare provider before starting it. Avoid isotretinoin and topical retinoids during pregnancy. Spironolactone can harm a developing fetus, so you should not become pregnant while taking it. Ask specifically about benzoyl peroxide and salicylic acid rather than assuming every pharmacy acne product is suitable. Pregnancy related hormone changes can cause acne, although breakouts often improve as pregnancy progresses.57

How long does treatment take?

For hormonal acne, improvement can begin within four to six weeks. Severe acne treatment can take two to three months to show improvement, so early changes are not the same as complete clearing. Keep using treatment as directed and ask when your clinician wants to review progress. If acne is worsening or not improving, arrange a review rather than repeatedly switching pharmacy products. An injection can reduce one painful cyst quickly, but managing recurrent severe acne takes a broader treatment plan.45

Make the appointment useful

Bring a list of acne treatments you have tried, because your bathroom shelf does not need to attend in person. Include current skincare, medicines and supplements. Some medicines and supplements, including anabolic steroids and whey, are associated with nodulocystic acne. Explain what you want treatment to change and how acne affects your daily life. Specific details, such as avoiding meetings or cancelling plans, help your dermatologist understand your priorities.23

Ask what each treatment is meant to do and what results to expect, so the routine has a plan rather than a supporting cast. Also ask about side effects, pregnancy safety and when to return. Knowing what to expect helps you make an informed treatment decision.35

What happens once the acne clears?

Ask for a maintenance plan rather than stopping everything when your skin clears. Many people continue with skincare and medicine applied to the skin, although the treatment can change. Without maintenance treatment, acne can return quickly. Early, effective treatment also helps limit scarring; in skin of colour, nodulocystic acne can leave dark marks and raised scars as well as other scar types.24
