Which strength is in your bottle?

A New Zealand version documented in 2005 contains 0.5% salicylic acid; check your own bottle for its current strength, ingredients and directions. Use the directions on your own bottle, because similarly named products do not have a contractual obligation to be identical. Salicylic acid comes in different forms and strengths for different conditions, so do not substitute directions from a cream, wart treatment or another acne wash.13

Is salicylic acid good for acne?

Yes. Salicylic acid is one of the topical treatments recommended in American Academy of Dermatology acne guidelines. It is used to help clear existing pimples and prevent new blemishes. Its ability to address blocked pores makes it relevant to blackheads and whiteheads. An acne wash is one part of skin care, not an entire treatment plan squeezed into a product name.34

Acne treatment can involve medicines that work in different ways. Benzoyl peroxide, topical retinoids and azelaic acid are also recommended options, but choosing a combination is different from adding every active ingredient to the same routine. If you already use an acne prescription, check with your prescriber before adding salicylic acid.14

What does salicylic acid do for acne?

Acne starts when pores become blocked with oil and skin cells. These plugs form whiteheads or blackheads, and trapped bacteria can trigger inflamed pimples. Salicylic acid is a keratolytic, meaning it helps loosen skin material. It softens keratin, a protein in the skin, and helps prevent clogging inside hair follicles. It also reduces swelling and redness and unplugs blocked pores so pimples can shrink.123

How do you use salicylic acid face wash?

Follow the bottle's washing directions. The steps below combine general salicylic acid precautions with gentle cleansing technique; they are not a replacement for Neutrogena's directions for your version.3

  1. 1When first using salicylic acid, try a small amount on one or two small affected areas for three days. If there is no reaction or discomfort, proceed with the bottle's directions.
  2. 2Apply the cleanser gently with your hands, using circular motions. Do not scrub. If washing a beard, reach the skin beneath the hair as well.
  3. 3Rinse the cleanser away thoroughly with lukewarm water. Avoid using a washcloth to scrub the skin.
  4. 4Gently blot dry with a clean towel rather than rubbing.
35

General acne cleansing advice is to wash once or twice daily, including after exercise, rather than repeatedly washing throughout the day. For this medicated wash, the bottle determines the permitted frequency. Start less often if salicylic acid causes dryness or irritation, and increase only as your skin adjusts and the directions allow. Keep the wash away from your eyes, nose and mouth; flush accidental contact with water for 15 minutes.23

What should you avoid using alongside it?

Unless your doctor directs otherwise, avoid using salicylic acid on the same area as abrasive cleansers, alcohol containing preparations, other medicated topical products, or cosmetics and soaps designed to dry or exfoliate skin. That includes benzoyl peroxide and topical retinoids. A prescribed combination can be appropriate, but adding these products yourself can cause substantial irritation. Choose water based or noncomedogenic skin creams; noncomedogenic means formulated and tested not to clog pores in most people.123

How long does it take to help?

Salicylic acid can take several weeks or longer to deliver its full benefit. Acne treatments generally take six to eight weeks to show a response. These are general treatment timelines, not a fixed clearing date for this wash. Judge progress by whether breakouts are becoming less frequent over time, not by the result of one wash. In beard acne, clearing commonly takes two to three months, and no reduction in breakouts after one month can mean a different treatment is needed.235

Who should be careful?

  • Do not apply salicylic acid to broken, infected, reddened or already irritated skin. Sensitive skin needs particular attention to new dryness, redness or discomfort.
  • If you are pregnant, trying to become pregnant or breastfeeding, discuss salicylic acid with your doctor before use.
  • Tell your care team about kidney or liver disease and any previous allergic reaction to salicylic acid or product ingredients.
  • Ask a clinician before using salicylic acid on young children. Children and teenagers with chickenpox or flu should not use it unless a doctor directs them to.

Salicylic acid treatment can increase sun sensitivity. Use protective clothing and sunscreen when sun exposure cannot be avoided, and avoid tanning beds.6

When should you stop or get medical help?

Report burning, crusting or peeling to your care team promptly. Get advice if mild irritation, redness or dryness continues or becomes bothersome. Stop salicylic acid and seek immediate medical help for hives, throat tightness, difficulty breathing, faintness, or swelling of the eyes, face, lips or tongue.36

See a healthcare professional if acne is worsening, causing scars or emotional distress, or includes deep cysts or substantial redness. Also seek help when self care and nonprescription treatment have not helped after several months. Persistent bumps are not always acne; a dermatologist can identify another condition and choose treatment that fits.27
