What are puffy eyes?

Temporary puffiness happens when fluid builds up in the thin tissues around your eyes and eyelids. Lasting bags can instead reflect excess skin or the fat behind the eyes moving forward. Dark circles are a separate concern: hollows can cast shadows, and extra pigment can darken the skin. Treatment depends on what is causing the puffiness, which is more useful than treating "eye bags" as a single item on a shopping list.12

What causes puffiness under your eyes?

Poor sleep, salt related water retention, allergies and irritating eye products can contribute to puffiness. Some persistent bags run in families because of fat pad prominence, bone structure or loose skin. With age, eyelid skin loses elasticity, and the fat that normally cushions structures behind the eyes can move forward into visible bags.12

Swollen eyelids are not always ordinary eye bags. Blepharitis, inflammation of the eyelids, can cause redness, burning and crusty eyelashes. A stye is a small, painful red bump caused by an infection in an oil gland; a chalazion is a painless lump that can form after a stye. An eye specialist can examine your eyelids and ask about your medical and family history to identify the cause.4

What remedies help with puffy under eyes?

For temporary puffiness without warning symptoms:

  1. 1Remove contact lenses if your eyes or eyelids are swollen.
  2. 2Lie down and place a washcloth soaked in cold water across your eyes for a few minutes. Wrap any ice pack in a towel to protect your skin.
  3. 3If makeup or eye cream seems to trigger swelling, stop using the suspected products for at least a couple of weeks to see whether symptoms improve.
  4. 4At night, aim for seven to nine hours of sleep and raise your head slightly with another pillow. Limit salty foods if fluid retention is contributing.

A cold washcloth can reduce swelling.1

Some eyelid disorders need warm compresses, medicines or ointments rather than cooling alone. Keeping eyelids clean can help prevent irritation and infection. If swelling includes a lump, crusting or burning, ask a pharmacist, optician or doctor which treatment fits rather than assuming every swollen eyelid needs the same remedy.34

What can you use for puffy eyes?

Eye creams containing caffeine or retinol sometimes improve the appearance of bags. Antihistamine eye drops help only when allergies are the cause. Use steroid eye drops only as prescribed. Do not use hemorrhoid cream around your eyes: it can irritate or damage the eyes, and prolonged hydrocortisone use can thin or discolor skin.1

How long should puffiness last?

Temporary swelling often settles within a day, and home remedies offer temporary relief. Have new swelling that lasts longer than 24 to 48 hours checked by an eye care professional. Longstanding bags caused by skin or fat changes are a different issue from a newly swollen eyelid.1

Seek an urgent GP or optician appointment, or use NHS 111 in the UK, if:

  • Your eyelid is red, hot, painful, tender or blistered.
  • Your eyelid suddenly droops, or you cannot open your eye or keep it open.
  • You have pain inside the eye, a very red eye, light sensitivity or changes in eyesight.
  • Eye symptoms come with a headache, nausea, vomiting, a high temperature or feeling generally unwell.
  • You think the swelling is an allergic reaction.
3

For symptoms that are not improving, keep returning or worry you, arrange a GP appointment. A pharmacist can advise on common eyelid problems and whether you need an optician or GP. An infection of the eyelid and nearby skin needs immediate medical care if it spreads behind the eye.34

What helps with lasting bags?

An oculoplastic surgeon, a specialist in surgery around the eyes, can assess whether bags, hollows or excess skin are the main problem. Fillers add volume to hollows beneath bags rather than removing the bags themselves. Fillers are temporary and can cause a bluish appearance or fluid buildup. Rarely, filler entering a blood vessel can cause permanent vision loss. People prone to eyelid fluid buildup, including those with inflammation, allergies or Graves' disease, might not be suitable candidates.2

Blepharoplasty is surgery to remove excess eyelid tissue, despite having a name that sounds like it should require a spelling test. It can correct puffy bags below the eyes and is usually performed for cosmetic reasons. Depending on the anatomy, surgery can remove excess skin and fat or reposition fat to address both a bag and the hollow beneath it. Removing fat alone can deepen hollows.256

Surgery brings its own recovery period. Eyelid swelling usually subsides within a few days, and bruising commonly resolves within two weeks. Swelling on the surface of the eye after lower eyelid surgery can last several weeks or months. Dry, gritty eyes can occur, especially if your eyes were already dry. Excessive skin removal can prevent full eye closure, and rare bleeding behind the eye can threaten vision.5

Before surgery, bring a list of all your medicines or your current repeat prescription and discuss existing dry eyes. After surgery, follow your surgical team's instructions for eye ointment, drops and swelling care. Alert your surgeon if vision remains severely impaired beyond 24 to 48 hours and does not clear with blinking.5
