Which one should you choose?

If your skin is very oily and texture is your main concern, ask about gel. If your skin is also sensitive or already peeling, make irritation part of the decision instead of choosing by oiliness alone. General acne guidance links gels with greater irritation than lotions; it does not give every tretinoin cream a gentleness advantage. The texture you can stand using matters, which makes texture a better tie breaker than loyalty to the word gel.12

These are the US Mylan formulations; other tretinoin products can have different bases and strengths.23

Both formats include 0.025%, so gel does not automatically mean a higher tretinoin concentration. Choose the prescribed strength separately from the texture.3

Why do tretinoin gels and creams feel different?

The vehicle is the base that carries the medicine onto your skin. Its composition affects drug delivery, skin properties and how pleasant the treatment is to use. A cream mixes water and oil phases, and the arrangement changes its finish: water in oil creams leave an oily film, while oil in water creams can be non greasy and moisturizing. Some creams are non greasy, so the word cream is not a formal declaration of greasiness, despite sounding like it belongs on a dessert menu. Those are general vehicle properties, not a prediction of exactly how a particular tretinoin tube will feel, since the product name is not a sample you can rub between your fingers.2

How should you start using tretinoin?

Use your prescription directions for the exact product. General topical retinoid guidance supports this starting approach:

  1. 1Wash gently and let your skin dry. Do not scrub.
  2. 2At night, apply a minimal amount in a thin layer across the acne affected areas, rather than only individual spots. Keep tretinoin away from your eyes, mouth and the corners of your nose.
  3. 3Start on alternate nights to limit irritation, following your prescriber's plan for increasing frequency.
  4. 4Use a light, non oily moisturizer if your skin becomes visibly scaly.
  5. 5Protect treated skin outdoors with sunscreen and protective clothing, and minimize sun exposure.

What should you expect?

Tretinoin is particularly useful for comedones, the blocked pores called blackheads and whiteheads. Continued use also reduces inflamed acne spots, although acne can appear worse initially. General acne treatments often cause dryness during the first two to four weeks, and convincing improvement takes several weeks or months. Arrange a review if spots have not cleared within six weeks rather than repeatedly changing formats on your own.15

When should you be careful or stop?

Do not use topical tretinoin during pregnancy. Eczema needs particular caution because tretinoin can cause severe irritation. Avoid applying tretinoin to sunburned skin until it has recovered. Stop and seek advice for severe irritation or a suspected sensitivity reaction. Ask your prescriber before combining tretinoin with other topical medicines, strongly drying cleansers or salicylic acid.135

See a doctor for deep, painful spots, scarring, hot and swollen skin, or acne affecting your mental health. Those problems need an acne treatment review, not just a different tube.4
