What actually changes?
| Feature | 0.025% tretinoin | 0.05% tretinoin |
|---|---|---|
| Active medicine | Tretinoin | Tretinoin |
| Concentration by weight | 0.025% | 0.05%, twice the concentration |
| Irritation | Lower concentration, but irritation can still occur | Higher concentration, with greater dose related irritation risk |
| Mylan acne cream base | Hydrophilic cream base | The same listed cream base |
| General application | A thin layer at bedtime, as prescribed | A thin layer at bedtime, as prescribed |
Cream, conventional gel and newer delivery formulations can behave differently, so compare the formula as well as the strength. In a 12 week acne trial, a particular 0.025% nanogel reduced acne lesions more and caused fewer local adverse events than a conventional 0.025% gel. The cream or gel base also affects tolerability, so the percentage is not the product's entire CV.4
Why is tretinoin available in different strengths?
Different strengths give prescribers options for balancing treatment needs against irritation. Tretinoin helps unclog pores and increases production of new skin cells; changing the percentage changes the concentration, not the medicine's identity. Dryness, redness, peeling and burning can limit treatment, and both the dose and the base influence irritation. A lower strength is therefore a meaningful treatment option, not simply a waiting room for a higher one.34
How do I know if my tretinoin strength needs changing?
- If acne is controlled, discuss continuing the current treatment rather than automatically increasing strength. Retinoids can be used for maintenance.
- If breakouts persist despite following the prescription, review the treatment plan with your dermatologist. A higher strength is one possible discussion, not the only change available.
- If irritation makes treatment difficult, discuss the amount, frequency and formulation before increasing strength. Your dermatologist can recommend less frequent use or moisturizer first.
Maintenance is a valid job for a retinoid, even if the word sounds less ambitious than 'upgrade'.5
How long should I give the current strength?
For acne, early improvement can appear after 2 to 3 weeks, but sometimes takes more than 6 weeks. Acne treatment plans commonly need at least 6 to 8 weeks before fewer breakouts become apparent. Fine wrinkle improvement takes longer: 3 to 4 months, sometimes up to 6 months. These are general treatment timelines, not separate deadlines for each strength.35
How do I use either strength?
- 1Wash your hands and treatment area with a mild cleanser, then let the skin dry as directed.
- 2At bedtime, apply a thin layer with clean fingertips, following your prescribed frequency. Keep tretinoin away from eyes, mouth and nose corners.
- 3Use moisturizer as directed. Ask whether applying moisturizer first would help if irritation is limiting treatment.
- 4During the day, protect treated skin with shade, clothing and broad spectrum, water resistant sunscreen of SPF 30 or higher.
When should I stop or contact my doctor?
Stop and contact your prescriber for a reaction suggesting sensitivity or chemical irritation. Call promptly for rash, hives or pain at the treatment area. Do not apply to sunburned or eczematous skin. Before changing strength, review other topical medicines, especially benzoyl peroxide and salicylic acid. Tell your prescriber if you are pregnant, planning pregnancy or breastfeeding; call if you become pregnant during treatment.13
