Which oils have a useful role?
| Oil or product | Practical choice |
|---|---|
| Olive oil | Avoid routine scalp application. Olive oil supports Malassezia growth in laboratory conditions. |
| Plain coconut oil | Avoid using it to grease an affected scalp. It also supports Malassezia growth in laboratory conditions. |
| Coconut oil in a prescribed scale removal preparation | Use as directed before other scalp treatments to help remove thick scale. |
| Tea tree oil shampoo | An alternative scalp treatment, but not the same as applying concentrated tea tree oil. |
| Lavender or concentrated tea tree face oil | Avoid strongly fragranced oils because they can irritate skin or cause allergic reactions. |
| Eucalyptus citriodora oil blends | Experimental formulations with laboratory antimicrobial activity, not an everyday scalp oil recommendation. |
Why can oils make flaking worse?
Malassezia is a yeast that normally lives on skin and plays a role in seborrheic dermatitis. It uses fats, and its breakdown of skin oils can release fatty acids involved in inflammation. Adding some oils to the scalp could support yeast growth rather than solve the flaking. Seborrheic dermatitis also affects the skin barrier, so flaking does not automatically mean the scalp needs more oil.246
Can you still oil your hair?
Hair oils can reduce friction and grooming damage, particularly in tightly coiled hair, which is a more practical ambition than the usual quest for shampoo advert hair. Keep oil on the hair lengths and ends instead of applying it to an affected scalp. An oil sold for hair care is not automatically a scalp treatment, even if the bottle has promoted itself to both jobs, which is a lot of ambition for packaging. If frequent washing is difficult for your hairstyle, a dermatologist can choose treatment forms that fit your washing routine, including foams or ointments.2
What about herbal and essential oils?
Some plant ingredients have antifungal activity, but much of the research involves laboratory testing rather than treating people. Experimental blends of Eucalyptus citriodora oil with Centella asiatica and Wedelia trilobata showed antimicrobial activity against microorganisms associated with dandruff. A herbal blend is a formulation, which makes "herbal" a rather incomplete recipe. For everyday care, choose an established scalp treatment rather than mixing essential oils at home. Plant ingredients can still have side effects and interactions.56
What should you use instead?
- 1If you have thick scalp scale and a prescribed descaling preparation, use that first. These treatments loosen scale so other treatments can reach the skin. Follow the prescribed contact time, which can be several hours or overnight.
- 2Wash the scalp with a medicated shampoo, such as ketoconazole, ciclopirox or selenium sulfide. A common schedule is twice weekly for at least a month. Follow the product directions and your treatment plan for contact time and frequency.
- 3For affected facial skin, wash gently with a fragrance free cleanser and rinse thoroughly. Wash the face twice daily, then apply a light, fragrance free moisturizer instead of a face oil.
- 4Once the scalp is clear, continue an antifungal shampoo once or twice weekly to reduce recurrence. Keep any prescribed treatment for inflammation within its instructed duration.
What should you expect from treatment?
Scale removal helps lift flakes; antifungal treatment reduces the yeast involved in seborrheic dermatitis. Facial antifungal creams are commonly used for 2 to 4 weeks, while scalp shampoo treatment usually needs at least a month. Adult seborrheic dermatitis often returns, so maintenance treatment is common. Coconut oil in a descaling preparation has a supporting role, not the job of controlling the condition by itself.34
When should you stop an oil or get help?
Stop a newly added oil if it irritates your skin or worsens the flare. Skin with seborrheic dermatitis is easily irritated, and irritation can trigger more inflammation. Get medical help for persistent symptoms, an uncertain diagnosis, or oozing and crusting, which can occur with infection. These adult treatment steps are not suitable for babies with cradle cap; babies need a separate care plan.137
