A blocked pore is not always a swollen pimple

A comedone is a plug inside a pore. A closed plug forms a whitehead, which can look white or flesh colored. An open plug forms a blackhead. Blackheads darken when their contents react with air, not because they contain dirt, despite the name giving dirt most of the blame.2

Inflamed pimples develop when trapped bacteria multiply and trigger swelling. Comedones can involve inflammation within the follicle without looking red or swollen on the surface. Some develop into inflammatory acne, but having clogged pores does not mean you must also have painful pimples.123

What makes pores clog?

  • More oil and a buildup of cells inside the follicle create the material that forms a plug.
  • Hormonal changes during puberty, menstrual cycles or pregnancy can make skin oilier. Family history also plays a role.
  • Oily cosmetics, heavy creams and greasy hair products can contribute. Humidity, sweating and repeated rubbing of the skin are other possible triggers.
  • Certain medicines, including steroids and testosterone, can trigger acne. If clogging began after a medication change, discuss the timing with your prescriber.

The forehead and nose are common sites because they have larger pores and oil glands. Comedonal acne also commonly affects the chin. Bumps concentrated along the hairline can be linked to waxy hair products; keeping those products away from facial skin is a useful change.36

How to care for clogged pores

  1. 1Cleanse gently morning and night with warm water and a mild cleanser. Wash after exercise, too. Avoid scrubbing and repeated washing.
  2. 2Use a pore clearing treatment as directed. Salicylic acid helps remove dead skin cells and unclog pores. Follow the product's instructions rather than adding extra applications.
  3. 3Moisturize with an oil free or noncomedogenic product. Choose those labels for makeup and other facial products, too.
  4. 4Apply sunscreen every morning. Choose broad spectrum, water resistant protection with SPF 30 or higher.
  5. 5Remove makeup before bed. Keep oily hair products off your face and clean makeup brushes regularly.

Noncomedogenic means formulated to avoid clogging pores, which is useful information hiding inside a word that looks designed for a spelling contest.7

Would a retinoid help?

Retinoids, medicines related to vitamin A, break up blackheads and whiteheads and help prevent new clogs. They treat the whole affected area rather than individual bumps. Adapalene is one option; retinoids are better suited to ongoing control than occasional spot treatment. Azelaic acid and benzoyl peroxide are other treatments used for comedonal acne. Antibiotics are usually prescribed for inflammatory acne rather than plugs alone.356

For cosmetic retinol, apply before bed; waiting 30 minutes after washing can reduce irritation. Do not use retinol or retinyl palmitate during pregnancy or breastfeeding. If salicylic acid dries or irritates your skin, alternate it with a gentle cleanser. Cheeks can be more sensitive to acne treatments, so every other day use can be more suitable there.67

How long should you give treatment?

With consistent care, you may notice improvement in eight to 12 weeks, while clearing can take several months. Continued treatment is often needed because comedonal acne can return. Avoid squeezing or extracting plugs yourself: damage to the follicle can cause more comedones, infection or scars. Persistent plugs can be assessed for professional extraction.13

See a clinician if several months of treatment have not helped, the bumps worsen, or you develop painful lumps or scars. Report medication side effects. Reduce or stop irritating exfoliation rather than pushing through excessive peeling or redness.146
