Why dry skin is easier to irritate
Your skin barrier is the outermost layer that helps keep moisture in and irritants out. Cold weather, harsh soaps, frequent washing and too much exfoliation can damage that layer, leaving skin dry, flaky and more vulnerable to irritation. People with eczema are particularly prone to reactions when their barrier is impaired. Facial skin can also be more sensitive to moisturizers than skin elsewhere. Moisturizers add moisture and reduce water loss, but the name is a job description rather than a character reference.245
Which ingredients can cause burning?
Urea and acidic preparations often sting on scratched or cracked skin. Urea helps skin retain water and shed rough surface skin, but it can also cause mild stinging, itching or a rash. Do not apply urea products to cracked, bleeding, infected or oozing skin, or areas with severe widespread redness. Fragrances, preservatives and plant extracts can cause allergic contact dermatitis, an itchy inflammatory skin reaction to something touching the skin. Burning alone does not tell you whether the cause is irritation or allergy.126
What moisturizer should you try instead?
Choose a fragrance free moisturizer without botanical extracts, and avoid exfoliating acids while your skin is dry and irritated. Ceramides help retain moisture, while ingredients such as petrolatum and dimethicone reduce water loss. For very dry skin, a cream or ointment usually retains more moisture than a lotion. Ointments are greasier and suit very dry areas that are not inflamed; creams and lotions are more suitable for inflamed areas. An expensive moisturizer is not necessarily more effective than a cheaper one, so the price tag is not a skin qualification.345
How to moisturize without adding more irritation
- 1Stop the moisturizer that causes burning. Ask a pharmacist to help you choose an alternative if your skin reacts.
- 2Wash with a fragrance free cleanser or moisturizing soap substitute. Keep showers lukewarm and limit them to five to 10 minutes.
- 3Pat skin gently, leaving it slightly damp. Apply the replacement moisturizer immediately after washing to trap water in the skin.
- 4Smooth moisturizer on in the direction hair grows rather than rubbing it in. For very dry skin, apply two to four times daily, or follow your doctor's instructions.
- 5Stop exfoliating dry, peeling skin. Using several antiaging products together can also increase irritation.
Before using a new product widely, test a small amount on your inner forearm twice daily for four to five days. Only expand use if the area remains free of redness, itching or other reactions. A product labeled hypoallergenic can still cause a reaction.7
Does a brief sting mean you are allergic?
Brief irritation can occur without an allergy, and a fleeting sting does not always require abandoning an otherwise tolerated moisturizer. Persistent burning or a developing rash is different: stop that product. Moisturizer can hydrate skin immediately, but maintaining hydration needs regular application. There is no reason to use burning as a measure of progress.12
When to get medical help
See a dermatologist if changing your routine does not help, or if dryness becomes painful, cracked, bleeding or very irritating. Repeated reactions can need professional patch testing to identify an allergen. If a prescribed skin product burns, ask the prescriber whether that reaction is expected before stopping it. If you use a prescribed topical corticosteroid for contact dermatitis, apply moisturizer first and wait around 30 minutes before the medicine, unless your clinician directs otherwise.2357
