What is happening in the skin?

Malassezia breaks down sebum, the oil made by your skin, releasing fatty acids that can trigger inflammation. The condition often affects oily areas such as the scalp, eyebrows, creases beside the nose and chest. The exact cause is not fully understood: the interaction between yeast, skin oils and individual susceptibility matters, rather than simply how much yeast or oil is present. Differences in the skin barrier also help explain why people react differently. Itch signals travel from the skin through nerves to the brain; the sensation is more than flakes sitting on the surface.345

Why does scratching not solve it?

Scratching can briefly suppress itch through its interaction with pain pathways, but repeated scratching damages the skin's protective barrier. Over time, scratched skin can become thickened. Breaking the skin also creates a risk of bleeding and infection. General itch relief measures include cooling the skin with a wet dressing or a lukewarm shower instead of scratching. These measures soothe the sensation; treating the underlying condition addresses its cause.15

How can you reduce the itch?

  1. 1For scalp symptoms, use a medicated dandruff shampoo. Antifungal options include ketoconazole, ciclopirox and selenium sulfide. General scalp treatment guidance is twice weekly for at least a month, but follow your particular product's directions or your clinician's instructions.
  2. 2Give the shampoo time to work before rinsing. Dandruff shampoos commonly need at least five minutes on the scalp; check the label for the product you use. If one does not help, consider a different active ingredient rather than another bottle with the same one.
  3. 3Reduce extra irritation. Avoid fragranced scalp products, including essential oils, and shampoos containing sodium lauryl sulphate if you have scalp eczema. Reduce styling products, then reintroduce them individually to identify irritants. Use a low or warm hairdryer setting rather than hot air.
  4. 4For affected facial or body skin, cleanse once or twice daily with a nonsoap cleanser. A clinician can help choose an antifungal cream and, when needed, a short course of treatment to reduce inflammation. Scalp and facial treatments are not interchangeable routines.

How long does relief take?

Symptoms usually improve quickly after treatment starts, but there is no single timetable for itch to disappear. Adult seborrheic dermatitis commonly returns, so maintenance treatment is often necessary. Treatment aims to control the itch and flakes, not persuade your scalp to honour a permanent ceasefire. Cold, dry weather, stress and alcohol based skincare products can trigger worse symptoms.134

When should you get medical help?

See a healthcare professional if the itch disrupts sleep or daily life, symptoms worsen, or treatments bought without a prescription are not helping. Pain, swelling or draining fluid need assessment because they can indicate infection. An uncertain diagnosis also deserves a check: psoriasis and contact dermatitis can resemble seborrheic dermatitis, and an itchy scalp has other possible causes. Stop using a cosmetic product that clearly aggravates irritation. For prescribed treatments, ask your prescriber about side effects and changes rather than adjusting the treatment yourself.13567
