Why can one area be oily and another dry?

Sebum is the oil made by your skin's oil glands. Too much makes skin shiny; too little can leave skin tight or scaly. Combination skin commonly has an oily forehead, nose and chin, known as the T zone, with drier cheeks. Your forehead and cheeks can need different care, even though skincare packaging would prefer one instruction for the whole postcode.34

The pattern is not necessarily permanent. Genetics, age, hormones and environmental conditions affect skin type, and your skin can change between summer and winter. Pregnancy, breastfeeding and some medicines can also change the pattern. Cold outdoor air and dry indoor heating can make dry areas worse without turning the rest of your face into dry skin.35

Is it combination skin or irritation?

After a gentle wash, notice how your forehead and cheeks look and feel over the next 30 minutes. Shine in one area and tightness or dryness in another fits combination skin. You can judge the pattern by how each area looks and feels, so blotting paper is optional rather than an entrance exam. Stinging, burning, itching or redness points toward sensitivity as well; sensitive skin can also be oily or combination.3

Check whether you are washing repeatedly to remove forehead shine or combining an acne cleanser with an acne treatment that stays on your skin. Excessive cleansing removes the surface oil film, increases water loss and can leave skin excessively dry. Combining acne products can also cause dryness and irritation. A flaky cheek does not automatically need an exfoliant: exfoliating dry, peeling skin strips away more moisture.15

How should I care for both areas?

  1. 1Wash gently morning and night with a mild, fragrance free cleanser, preferably a soapless option. Use warm water, rinse thoroughly and pat dry. Cleanse after exercise too, rather than scrubbing away oil.
  2. 2Moisturize the flaky cheeks while the skin is still damp. A cream holds more moisture than a lotion; an ointment is thicker still. Choose a noncomedogenic product, meaning one designed not to block pores.
  3. 3Treat the forehead separately. Skip moisturizer there if the area does not need it, or choose a lighter gel. Moisturizer can go on your cheeks without going on your forehead, because buying one pot does not create a contractual obligation to cover your whole face.
  4. 4Avoid exfoliating the flaky areas and avoid fragranced products. If your skin is sensitive, benzoyl peroxide and salicylic acid can cause irritation rather than being the right response to shine.
  5. 5During the day, use broad spectrum sunscreen with SPF 30 or higher. Moisturizing only selected areas does not mean protecting only selected areas from the sun.

When should I get the flaking checked?

Greasy scales, itching or a rash around the eyebrows, sides of the nose or scalp can fit seborrheic dermatitis, a condition that causes a scaly rash. Affected facial skin can be oily in some places and dry in others. On darker skin, the rash can appear darker or lighter than surrounding skin rather than red. If you have dandruff alongside flaky, itchy facial patches, arrange a medical assessment instead of treating everything as ordinary dryness.26

See a dermatologist if moisturizing does not help, or the cheeks become painful, cracked, bleeding, stinging or very irritated. If forehead shine comes with blackheads or whiteheads, those are clogged pores, not just excess surface oil. Avoid picking or squeezing them. Tell your prescriber about medication side effects if you are already treating acne.57
