Why the contents stay trapped
A whitehead forms when excess oil and dead skin cells plug a pore's opening. The medical name is closed comedo, meaning a closed pore. Unlike a blackhead, which has a widened opening, a whitehead's contents remain covered by skin. Closed comedones need an opening before their contents can be extracted; that is a reason to leave extraction to a professional, not to reach for a needle.134
Is the bump actually a whitehead?
A whitehead is usually a small raised bump that is white or flesh colored. A bump filled with pus is a pustule, while an inflamed pimple without pus is a papule. A larger, deep, tender lump can be an acne nodule or cyst. Nodules do not contain pus, so a hard, painful bump is not necessarily holding something that can be squeezed out. The distinction changes what you should do next.3
What to do instead of squeezing
- 1Leave the bump alone. Picking whiteheads can lead to more whiteheads and acne scars, rather than clearing the original blockage.
- 2Wash gently morning and night, and moisturize regularly. Do not scrub: scrubbing can irritate skin and make acne worse.
- 3Choose an acne treatment with help from a pharmacist if needed. Salicylic acid and benzoyl peroxide are options available without a prescription.
- 4Choose skincare and makeup labelled noncomedogenic, meaning designed not to clog pores. That label is more useful than a product name that sounds like it requires a laboratory clearance.
Salicylic acid dissolves dead skin cells to help prevent follicles from clogging. Retinoids, medicines related to vitamin A, break up whiteheads and help prevent clogged pores. Benzoyl peroxide targets surface bacteria that can aggravate acne; lower concentrations and wash formulas are less irritating. Benzoyl peroxide can cause dryness, and retinoids can cause peeling or changes in skin color. Using a retinoid every other day or alongside moisturizer can reduce those effects.6
How long does treatment take?
An individual whitehead can go away on its own, sometimes within seven days. Treating repeated whiteheads is a longer job: with consistent care and treatment, improvement can start in eight to 12 weeks, while clearing comedonal acne can take many months. A routine is treatment, not a product demonstration with an immediate reveal. If medication causes side effects, let your healthcare provider know.16
What if the bump is deep and painful?
Do not try to pop a deep, painful pimple. Apply a clean, warm, damp washcloth for 10 to 15 minutes, three times daily, using a clean cloth each time. This can help the bump move closer to the surface so it can heal. A hydrocolloid acne patch can protect the skin and support healing. Wear broad spectrum sunscreen with SPF 30 or higher: inflammation can leave dark marks, and some acne ingredients increase sun sensitivity.7
When to get help
See a GP or dermatologist if treatment is not helping, you have lots of spots, or bumps are deep, painful or causing scars. Hot, swollen, red skin can signal an infection; redness can be harder to see on brown and black skin. Get help for those symptoms, or if acne is affecting your mental health. For persistent closed pores, a dermatologist or licensed esthetician can use sterile instruments for professional extraction.15
