Does one pimple mean you have acne?

Acne can cause just one spot or many, so the distinction is not simply "one pimple versus lots of pimples". Mild acne can involve only a few small spots near the skin's surface. People also use "pimples", "spots" and "zits" to mean acne generally. The everyday names overlap, which gives one small bump a surprisingly large vocabulary.123

What causes acne and pimples?

Acne starts when oil and dead skin cells plug a hair follicle, the small opening associated with a hair and oil gland. Bacteria normally present on skin can grow within the blocked follicle and contribute to inflammation, meaning swelling, redness and pain. Inflamed spots often develop after the wall of a blocked follicle breaks, although they can also arise from skin that previously looked normal.47

Acne is not caused by dirty skin and is not contagious. Blackheads darken because their contents meet the air, not because the pores contain dirt. Acne is not a hygiene report card, despite the amount of washing its name seems to invite. Hormones, family history and certain medicines can contribute to acne; it can continue into adulthood rather than ending with the teenage years.24

How should you treat ordinary pimples?

  1. 1Start with gentle cleansing once or twice daily. Avoid hard scrubbing, strong cleansers and alcohol based toners, which can irritate skin and worsen acne.
  2. 2Match treatment to the spots. For small inflamed pimples, a face wash containing benzoyl peroxide or salicylic acid is an option. For blackheads and whiteheads, adapalene, a retinoid that helps unclog pores, is an option; a benzoyl peroxide wash can also help reduce acne bacteria.
  3. 3Choose skincare, makeup and sunscreen labelled oil free or noncomedogenic, meaning formulated not to clog pores. Apply an oil free moisturizer if your skin feels dry.
  4. 4Give treatment six to eight weeks to show improvement. Do not squeeze or pick spots. If pimples are not improving after that period, see a dermatologist rather than repeatedly switching products.

How does cystic acne differ from a pimple?

An ordinary papule or pustule sits closer to the surface. Cystic acne forms painful, pus filled bumps deep under the skin, while nodules are deep but solid. Both can leave permanent scars. Get a dermatologist's help for deep, painful breakouts instead of relying on shop bought acne products. Treatment can include prescription creams or gels and oral medicines. Picking or opening a cyst increases the risk of scarring and bacterial infection.356

How long does a cystic acne pimple last?

Most individual acne spots last less than two weeks, but deeper bumps can persist for months. Clearing acne cysts with treatment can take three months or longer. That is different from the six to eight weeks needed to see initial improvement with ordinary acne treatment. A flat dark or red mark left after a spot heals is not the same as an active pimple; colour changes after a cyst can take more than a year to fade.136

Seek medical help if you have signs of a skin infection, developing scars, or acne that affects your confidence or daily life. Even mild acne can scar, so you do not have to wait for widespread breakouts before asking for treatment.26
