Why can the combination be irritating?
Adapalene is a retinoid, a medicine related to vitamin A that changes skin cell turnover to help unblock pores and reduce spots. Salicylic acid also helps prevent clogged pores and breaks down blackheads and whiteheads. Both are acne treatments, and acne guidelines support using treatments with different mechanisms together. That does not mean every combination belongs in a routine: salicylic acid precautions specifically advise against combining it with topical retinoids unless a doctor directs you to.245
Some salicylic acid cleansers are gentle enough for daily use on their own. That is not a reason to keep one when starting adapalene. A gentle cleanser is a useful part of an acne routine, even if 'gentle' has a less impressive marketing department.67
What should my routine look like?
- 1Wash gently with a mild cleanser. Use warm water rather than hot water, avoid scrubbing, and pat your skin dry. Gentle cleansing twice daily is standard acne care.
- 2Apply adapalene to clean, dry skin as directed for your product. For cream and gel, apply a thin layer across the area where you get acne, not just individual spots. Keep it away from your eyes, lips and the inside of your nose.
- 3Follow with moisturiser. For adapalene cream and gel, wait until the treatment has been absorbed before applying moisturiser or cosmetics.
- 4Protect your skin during the day. UK adapalene cream and gel advice specifies SPF 50 sunscreen plus protective clothing or a hat, including on cloudy days. Avoid sunbeds.
Adapalene cream and gel are usually applied once daily before bed; US Differin Gel allows morning or evening use. Applying extra or using it more than once daily will not produce faster results. If you are sensitive to treatment, UK cream and gel advice allows starting two to three times weekly and increasing towards nightly use as tolerated.12
Choose skincare labelled noncomedogenic, meaning designed not to clog pores; the useful part of the bottle is not always the biggest print.7
Can I use salicylic acid in the morning instead?
Separating the products does not override the advice to avoid salicylic acid alongside adapalene. If your clinician approves a salicylic acid cleanser, follow its label rather than inventing a contact time or increasing the frequency. Do not use salicylic acid on irritated, infected or reddened skin. Avoid adding abrasive cleansers, drying products or other exfoliating acids to the same routine.124
What should I expect from adapalene?
Dryness, redness, scaling and burning or stinging can occur early in treatment. With US Differin Gel, irritation usually peaks around weeks two to four before easing. Some people see improvement after a couple of weeks, but most need at least a couple of months. Lack of irritation does not mean the gel is failing, and early irritation is not a reason to add another acne treatment.12
When should I stop or get help?
For severe or persistent irritation, stop adapalene and speak to your doctor before restarting. Do not apply it to broken, sunburned or eczema affected skin. If salicylic acid causes moderate or severe irritation, unusual warmth or redness, stop using it and seek medical advice. See a GP if acne is not improving with treatment, causes deep painful spots or scars, or affects your mental health.1247
If you think you could be pregnant, stop adapalene and contact your clinician. Check with your GP before using adapalene while breastfeeding, and discuss salicylic acid use during pregnancy or breastfeeding with your doctor.24
