Why can acne look worse at first?
Salicylic acid is a beta hydroxy acid, or BHA, that helps unclog pores and break down blackheads and whiteheads. Acne can worsen during the first few days of treatment while skin adjusts. That early worsening is what people commonly mean by purging here, rather than a separate diagnosis.134
New spots alone cannot tell you whether you are experiencing early worsening or irritation, so the word purging has been given rather more responsibility than it can handle. Look at the accompanying symptoms and whether your skin is becoming increasingly uncomfortable, not just the number of spots.12
What points to irritation instead?
Dryness, stinging, burning, redness or unusually warm skin are reasons to reassess use, not signs you need more exfoliation. Salicylic acid can irritate healthy skin, and excessive exfoliation can remove structures that help skin retain water and healthy fats. Sensitive skin needs particular care because products can cause stinging or burning.234
How should you start or adjust your routine?
- 1Test a small amount on one or two small treatment areas for three days before wider use. Continue as directed only if there is no reaction or discomfort.
- 2Follow your product's label rather than a schedule for another BHA. Start less often if needed, and increase only after your skin adjusts. Reduce frequency if dryness or irritation develops.
- 3Keep application off broken, red, swollen, irritated or infected skin. Do not exfoliate sunburned skin or open wounds.
- 4Follow exfoliation with moisturizer. Use gentle cleansing rather than scrubs or abrasive tools.
- 5Use SPF 30 or higher sunscreen daily. When outdoors, choose broad spectrum, water resistant protection and seek shade.
Check the rest of your routine before adding BHA. Retinol, prescription retinoids and benzoyl peroxide can make skin more sensitive or cause peeling; adding exfoliation can worsen dryness or breakouts. Do not combine salicylic acid with other medicated topical products on the same area unless your doctor directs you to.24
A simple routine is easier to keep consistent, even if it gives the bathroom shelf less to do. Avoid repeatedly switching acne treatments, and do not pick or squeeze spots: that prolongs clearing and increases the risk of scars and dark marks.35
How long should you wait?
Salicylic acid can take several weeks or longer to deliver its full benefit. Acne treatment more generally can take several weeks to a few months before you see a difference. Those treatment timelines are not a reason to tolerate ongoing irritation or assume every later breakout is purging. If breakouts continue despite a consistent, gentle routine, see a dermatologist.15
When should you stop or get help?
Stop applying BHA to irritated skin. Get medical advice for moderate or severe irritation, unusual warmth or redness, or stinging that persists. Stop use and seek immediate medical help for hives, facial swelling, throat tightness, breathing difficulty or feeling faint. If you are pregnant or breastfeeding, discuss salicylic acid use with your doctor before starting.14
