Do you need to start every night?
Not necessarily. General retinoid guidance suggests starting every other night; topical acne treatment guidance also allows once or twice a week initially, building toward daily use if tolerated. These are starting approaches to discuss with your prescriber, not a reason to replace your prescription instructions. There is no fixed number of weeks after which everyone should switch to nightly tretinoin.234
If irritation develops, your dermatologist can recommend fewer applications, a smaller amount or moisturizer before the medication. Make those changes with their help. Previous irritation does not automatically mean a retinoid allergy or rule out future use. For acne, retinoids can also serve as maintenance treatment after breakouts are controlled.4
How to apply tretinoin at night
Use a gentle cleanser, which has a useful job without needing to call itself a resurfacing experience.5
- 1Wash your hands and the treatment area with a mild cleanser and water. Let your skin dry as directed.
- 2Use clean fingertips to spread a thin layer across the affected area. For acne, treat the affected area rather than only individual spots.
- 3Keep tretinoin away from your eyes, mouth, ears and nose corners. Do not apply it to sunburned skin or areas affected by eczema.
- 4Use moisturizer as agreed with your prescriber. Applying moisturizer first is one option a dermatologist can suggest to reduce irritation.
The supporting routine can stay modest, even if the bathroom shelf would prefer a larger cast. For acne, avoid scrubbing and washing more than twice daily. A fragrance free, water based moisturizer is an option for dry skin. During the day, use sunscreen with broad spectrum protection, SPF 30 or higher and water resistance, alongside shade and protective clothing.457
Check with your prescriber before adding other topical medicines, including benzoyl peroxide or salicylic acid. Prescribed combinations are different from assembling your own combination. Avoid cosmetics containing lots of alcohol or menthol, which can sting.14
How long before you see results?
Acne can worsen during the first 7 to 10 days. Early improvement can appear within 2 to 3 weeks, but allow at least 6 to 8 weeks to assess fewer breakouts; acne treatments can take up to 3 months to work. A treatment review around 12 weeks helps decide whether to continue or change the plan. Fine wrinkle improvement takes longer, typically 3 to 4 months and sometimes up to 6 months. Do not increase frequency beyond your prescription to chase faster results.1247
When should you be careful?
Very dry, sensitive or already inflamed skin needs a prescriber discussion before starting. In darker skin, irritation can trigger dark marks, called hyperpigmentation, making a gradual start particularly useful. Do not use tretinoin during pregnancy or when planning pregnancy. If you become pregnant, discontinue it and contact your doctor. If breastfeeding, discuss an alternative before use.236
Tell your doctor about severe or persistent peeling, burning, redness or swelling. Call immediately for a rash, hives or treatment area pain. Do not treat substantial irritation as something to push through. Seek medical help for acne that is deep, painful, scarring or affecting your mental health rather than simply applying tretinoin more often.15
