Why is there no head?

Oil, dead skin cells and bacteria can become trapped in a pore, causing inflammation, meaning swelling. When that swelling extends deep into the skin, a nodule or cyst develops. You can sometimes feel the bump more easily than you can see it. Nodules usually have no whitehead or blackhead at their centre.134

An acne cyst contains pus and feels softer. A nodule does not contain pus and feels harder, like a knot beneath the skin. A smaller raised pimple without pus is called a papule. Having no head alone does not identify which type you have; a clinician can examine the lump.1456

What can you do at home?

For an isolated deep pimple, gentle care can help while you arrange treatment if needed. Confirmed nodular or cystic acne is not something to manage with shop bought spot creams alone. Topical acne products work better when a pimple is nearer the skin's surface.135

  1. 1Wash gently with a mild cleanser and lukewarm water, morning and evening. Use your fingers rather than scrubbing the bump.
  2. 2Apply a clean, warm, damp washcloth for 10 to 15 minutes, three times daily. This can help a deep pimple move towards the surface. Use a clean cloth each time, and do not squeeze it if a head appears.
  3. 3If using an acne product, apply a thin layer rather than repeatedly coating the lump. Options include adapalene, azelaic acid, benzoyl peroxide or salicylic acid; too much can irritate the skin.
  4. 4Protect exposed skin with broad spectrum, water resistant sunscreen of SPF 30 or higher. Seek shade and use protective clothing too, because inflammation and sun exposure increase the risk of dark marks.

A hydrocolloid acne patch can protect the skin and help healing, but it is a dressing, not a miniature clinic. Do not use a patch as a substitute for treatment of a persistent deep lump.13

If your skin feels dry, use a fragrance free, water based moisturiser. Remove makeup before bed. Choose products labelled noncomedogenic, meaning less likely to clog pores, rather than recruiting an entire bathroom shelf. Extra washing will not remove the blockage beneath the skin and can make irritation worse.2

How long will the lump last?

Acne nodules can last for weeks or months. Treatment for cystic acne can take three months or more to clear the cysts. Those are different timelines: one describes how long a lump can persist, and the other describes treatment progress. Do not wait months to seek help for deep, painful acne, particularly if more lumps appear or scars are developing.15

When should you see a doctor?

Book a GP or dermatologist appointment for suspected nodules or cysts, multiple deep painful bumps, scarring, or acne that is not improving with self care. Seek medical advice for signs of infection such as hot, swollen, red skin; redness can be harder to see on brown or black skin. Acne affecting your mental health is also a reason to get help.26

A dermatologist can select prescription creams or gels and, when needed, oral medicines such as antibiotics, hormonal treatments or isotretinoin. For a very large, painful or lingering lump, a corticosteroid injection can reduce inflammation and speed healing. A clinician can also drain an acne cyst when appropriate; opening it yourself increases the risk of infection and scarring.15
