A cyst healing is not the same as acne ending
Individual cysts can heal, sometimes leaving scars, while the underlying acne continues to produce breakouts. There is no dependable deadline for untreated cystic acne to disappear. Hormones settling with age can help, but severe acne involving both nodules and cysts commonly continues into adult life.123
Cystic acne causes painful, pus filled lumps deep beneath the skin. Nodules are similar deep lumps but are firmer and more solid rather than filled with fluid. Both need medical attention because both can scar; a small surface pimple and a deep, painful lump are not the same treatment problem.12
When should you see a doctor?
Book a GP or dermatologist appointment for deep, painful lumps, worsening acne or developing scars. You do not need to spend several months testing shop bought treatments first when the breakouts are cystic. A shelf of acne products is not a substitute for specialist care, even if the packaging looks very organised.145
Get medical help if the skin becomes hot, swollen and red, which can signal infection; redness can be harder to see on brown and black skin. Acne affecting your mood or causing emotional distress is also a reason to seek help. A clinician can confirm the diagnosis because some other conditions resemble acne.46
What treatment can clear cystic acne?
Treatment aims to clear existing acne, stop new breakouts and prevent scars. A dermatologist can combine prescription treatments, but creams and gels alone are usually ineffective for severe acne involving nodules and cysts. Oral isotretinoin, a vitamin A related medicine taken by mouth, is a treatment for this severe form. Hormonal treatments are another option for some women.136
For an individual large, painful cyst, a dermatologist can use a corticosteroid injection to shrink it quickly or perform drainage when appropriate. Do not squeeze, pierce or pick the lump yourself: doing so increases the risk of infection and scarring.15
Isotretinoin requires close monitoring because of its side effects. It must not be taken during pregnancy because it causes severe birth defects. Treatment includes regular blood tests and pregnancy prevention requirements for people who can become pregnant.5
How long does treatment take?
Acne treatment generally takes at least six to eight weeks before you start seeing fewer breakouts. Clearing acne cysts can take three months or longer. For severe acne involving nodules and cysts, an isotretinoin course takes at least five months, and some people need further courses. These are treatment times, not a countdown for untreated acne.136
What can you do while waiting for treatment?
- 1Wash gently with a mild cleanser and lukewarm water when you wake up and before bed, and after sweating. Use your fingers rather than a washcloth or sponge.
- 2Remove makeup before bed and choose skincare and makeup labelled noncomedogenic.
- 3Apply an oil free moisturizer if your skin feels dry.
- 4Use prescribed treatments exactly as directed. Applying extra medication will not clear acne faster.
- 5Tell your dermatologist about irritation. They can adjust a retinoid's amount, frequency or moisturizer order; do not change the prescription routine yourself.
Noncomedogenic means formulated not to clog pores, a useful idea wearing a word far too large for a bottle.4
