Is it a dark mark or a scar?
- A flat patch of darker colour after a breakout is usually a pigment mark. Colour changes often improve with time.
- An indentation is an atrophic scar, meaning a scar with lost tissue. Narrow deep pits, sharply edged depressions and broader rolling depressions need different treatment approaches.
- A raised scar involves excess collagen, the protein that supports skin. Raised scars and keloids require different treatments from depressed scars.
Discolouration can also sit on top of a true scar. Fading the colour does not necessarily remove the indentation or raised tissue underneath. Treatment is chosen for the scar's depth and shape, not for how ambitious the packaging sounds.23
What can lactic acid actually improve?
Lactic acid is an alpha hydroxy acid, or AHA, that exfoliates the skin's surface. Removing accumulated dead skin cells can make rough skin feel smoother. Some lactic acid products also help moisturise skin, while AHA creams and lotions can help irregular pigmentation. These are useful surface benefits, not the same thing as correcting deeper scar tissue.67
A 92% lactic acid peel has shown improvement in the texture and colour of pigmented acne scars in skin types IV and V. That is a strong peel, not a starting strength for home skincare. Chemical peels have stronger support for pigmentation after acne than for indented scars. A dermatologist should choose peel strength for your skin type and the severity of scarring.1
How should you start using lactic acid?
- 1Choose a home AHA product rather than a strong peel. General beginner guidance sets a maximum starting concentration of 10% to 15% AHA; that is a ceiling, not a strength you need to reach.
- 2Start every other day. Increase towards daily application gradually only if your skin tolerates the product.
- 3Apply sunscreen every morning. AHAs can increase sun sensitivity.
- 4If irritation develops, stop rather than increasing the frequency. Mild irritation is a recognised AHA side effect.
How long does improvement take?
Pigment changes after acne can take months or longer to improve. There is no fixed lactic acid timetable for scar improvement. For professional treatment, mild chemical peels commonly involve 3 to 5 sessions spaced 2 to 4 weeks apart. That schedule describes mild peels generally, not a promised result from a home lactic acid product.35
When is a dermatologist the better next step?
Get an assessment for persistent indentations, raised scars or scars that bother you enough to want treatment. Control active acne before starting scar procedures, and avoid picking or squeezing breakouts to reduce further scarring. Tell your dermatologist about acne medicines used within the past two years, because some affect the safety of scar treatments. A treatment plan also has to fit your budget and downtime, because the appointment does not come with a pause button for the rest of your week.235
