What does dryness look like?

Tightness and flaking are signs of dry skin. Sensitive skin stings easily, particularly during or after cleansing, and is more likely to be dry. Salicylic acid also causes irritation and temporary stinging, especially on cracked or damaged skin. Do not apply it to broken, red, swollen, irritated or infected areas.345

How to use salicylic acid without making dryness worse

Washes, pads and creams have different directions, so follow your product's label for application and frequency rather than using a universal morning or evening rule. The package directions deserve a read, even if skincare packaging would rather discuss radiance.1

  1. 1Test a small amount on one or two small treatment areas for three days. If there is no reaction or discomfort, proceed as directed.
  2. 2Start less frequently, then increase use as your skin adjusts, within the label's directions. Reduce frequency again if dryness or irritation develops.
  3. 3Wash gently rather than scrubbing. Rinse thoroughly and gently pat dry. Follow the salicylic acid label's instructions for preparing your skin.
  4. 4Use moisturizer daily to help tolerate the treatment. After washing, applying moisturizer while some water remains on your skin helps trap moisture; follow separate preparation instructions for your salicylic acid.

For skin prone to acne, look for a moisturizer labelled "oil free," "noncomedogenic" or "won't clog pores." These descriptions mean the moisturizer is unlikely to cause acne. Moisturizer is useful even when your skin feels oily, because acne treatments can still leave it dry and irritated. A moisturizer helps retain water in skin, which is a more useful job than sounding expensive.2

Check the rest of your routine

A salicylic acid cleanser is not automatically exempt from causing dryness. Washing too much removes the surface oil film, allowing more water to escape from skin. Cleansers themselves can also cause red, dry, chafed skin. Using an acne cleanser alongside acne products left on the skin calls for particular caution: too much treatment causes excessive dryness and irritation.3

Avoid abrasive cleansers, alcohol containing skincare and other topical medicines such as benzoyl peroxide or tretinoin on the same treated areas unless your doctor directs the combination. Before starting, discuss prescription treatments and pregnancy, planned pregnancy or breastfeeding with your doctor.5

Does dryness mean salicylic acid is working?

Dryness is a side effect, not the treatment goal. Salicylic acid helps unplug blocked pores, but evidence of its acne benefit is limited, so it has a conditional recommendation in acne guidelines. Full benefit can take several weeks or longer. For pimples with a pus filled centre, see a dermatologist if treatment has not helped after six to eight weeks. Deep acne nodules or cysts need dermatological treatment rather than more aggressive washing.167

When should you stop or get help?

Contact your doctor if irritation or stinging is severe or does not go away. Stop salicylic acid and seek immediate medical help for hives, itching, throat tightness, difficulty breathing, faintness, or swelling of the eyes, face, lips or tongue.5
