What is purging in skincare?
Purging describes temporary breakouts associated with faster skin cell turnover, meaning faster replacement of skin cells. It can happen after a facial. Tretinoin, a vitamin A medicine that increases new skin cell production and unclogs pores, can also make acne worse before it improves. That does not make every breakout after a new product a purge.13
Could it be irritation or another breakout?
Retinoids can cause dryness, redness, peeling and irritant dermatitis, which is inflammation caused by irritation. Exfoliating acids can also irritate skin when overused. These symptoms deserve attention rather than being taken as proof that a product is working. Products that make skin greasy can contribute to acne, and even products labelled noncomedogenic, meaning they are intended not to clog pores, cause breakouts in some people.2456
Trying too many products or switching treatments every few days can worsen acne. Start with one or two acne products and give them time to work. A routine does not have to be complicated, even if the bathroom shelf has other ambitions.25
How long should it last?
With tretinoin, acne commonly worsens during the first 7 to 10 days. Acne sores should improve after 2 to 3 weeks of regular use, although improvement sometimes takes more than 6 weeks. Continue as directed unless side effects require medical advice.1
For acne treatment generally, early improvement can appear in 4 to 6 weeks, while clearing can take two to three months or longer. Some topical retinoids take 12 weeks or longer before improvement appears. These are treatment timelines, not a deadline that makes every worsening breakout acceptable.24
Breakouts after a facial should subside within a week or two. Facials can include cleansing, exfoliation and hydration, so 'a facial' names a bundle rather than one very busy product.3
How to care for your face now
- 1Wash gently morning and evening, and after sweating. Use a gentle cleanser without scrubbing.
- 2Apply acne treatment according to its directions. Spread a thin layer across the acne prone area, rather than treating only visible spots. Use prescribed tretinoin at bedtime exactly as directed.
- 3Use moisturizer to help manage retinoid dryness. If irritation develops, ask your dermatologist whether to apply moisturizer first, use less medication or use it less often. Make prescription changes with their help.
- 4During the day, use sunscreen with SPF 30 or higher, broad spectrum protection and water resistance. Seek shade and wear protective clothing, particularly when using retinoids or exfoliating acids.
When to get help instead of waiting
With tretinoin, call your doctor immediately for rash, hives, itching, or pain at the application site. Report severe or persistent burning, peeling, swelling or blistering. Do not add other topical medicines unless your doctor directs you to. Tell your prescriber if you are pregnant, planning pregnancy or breastfeeding; call them if you become pregnant during treatment. Retinoids can also worsen eczema.14
See a doctor for deep, painful spots, scarring, acne that is not improving with treatment, or acne affecting your mental health. Hot, swollen, red skin can indicate infection and also needs assessment. Persistent unfamiliar breakouts deserve a diagnosis: conditions such as perioral dermatitis and folliculitis can look like acne but need different treatment.67
