Does it look like heat rash?

Heat rash, also called prickly heat or miliaria, develops when sweat ducts become blocked and sweat gets trapped. Typical signs are small raised spots, an itchy or prickly feeling and mild swelling. The spots can look red on white skin or grey or white on brown or black skin. Heat rash can spread across your body, but you cannot pass it to someone else.1

In adults, heat rash often affects the upper body, scalp, neck and skin folds, especially where clothing rubs. Children commonly get it on the trunk and in neck, armpit or groin folds. Heat, humidity, intense exercise and clothing that traps sweat all contribute.3

What to do at home

  1. 1Move somewhere cooler. Use a fan or air conditioning when possible, and reduce activities that make you sweat heavily.
  2. 2Change out of wet or tight clothing. Choose loose cotton clothing and use lightweight bedding.
  3. 3Take a cool shower or bath. Avoid perfumed shower gels and excessive soap, which can irritate the rash.
  4. 4Apply a cold, damp cloth or an ice pack wrapped in a towel for up to 20 minutes to calm itching.
  5. 5Tap or pat itchy skin instead of scratching. When removing sweat, blot gently with a clean cloth rather than wiping.

Loose cotton clothing helps keep skin cool, so the dress code is less ambitious than the skincare aisle.1

Which lotions and home remedies help?

If cooling does not settle the discomfort, a pharmacist can advise on calamine lotion, antihistamine tablets or hydrocortisone cream. Calamine can relieve discomfort but also dries the skin. Children under 10 need a doctor's advice before using hydrocortisone for heat rash.13

For dry, irritated skin rather than prickly heat alone, use a mild cleanser and a fragrance free moisturizer after bathing. Colloidal oatmeal in a moisturizer can soothe dry itching. Avoid piling on heavy moisturizers if the rash has pimples around hairs: excessive emollient use can contribute to folliculitis, inflammation of hair follicles.245

Skip apple cider vinegar because its acidity can worsen irritated skin. Aloe vera is not a reliable treatment for dry itching, and fragrances or preservatives in aloe products can cause further irritation. These are general itch precautions, not reasons to add another treatment to heat rash.5

How long should it take to clear?

Most heat rash resolves within one or two days after moving to a cooler environment; allow a few days for improvement. To reduce repeat episodes, sleep in a cool, ventilated room and exercise during the coolest parts of the day or indoors with air conditioning. A fan helps keep skin cool, without requiring a bottle to have an opinion about it.34

When should you get medical help?

See a doctor if the rash worsens or does not improve after a few days, or if your baby has a rash and you are worried. Itchy, tender pimples around hairs or deeper painful boils can indicate folliculitis rather than heat rash. A rash triggered by an irritant or allergy needs the trigger identified and avoided.1246

Heavy sweating with rapid breathing and a fast, weak pulse can signal heat exhaustion. Confusion, dizziness, nausea or a rapid, strong pulse in the heat can signal heat stroke: get medical help right away. Do not treat these symptoms as just a skin problem.7
