Why does your face get hot or red?

Flushing happens when blood vessels in the skin widen, increasing blood flow. Your cheeks, neck or upper chest can feel warm and turn red or purple. On darker skin, the change can look like slight darkening rather than obvious redness. Alcohol related flushing can take a couple of hours to fade as the effects wear off.34

Acetaldehyde, a breakdown product of alcohol, contributes to flushing. Some people have a genetic difference in the enzyme that clears acetaldehyde, so it builds up even after small amounts of alcohol. This is particularly common in people of northeastern Asian ancestry. Histamine or tyramine in fermented drinks such as beer, sherry and wine can also provoke flushing.13

Does alcohol cause rosacea?

Alcohol is a flushing trigger for people with rosacea, but having rosacea does not mean someone drinks heavily. People who never drink develop rosacea too. Drinking is associated with a higher risk of developing rosacea, but whether alcohol directly causes it remains uncertain. In research involving women, risk increased with the amount consumed, and white wine and hard liquor were associated with greater risk.5

Repeated flushing, redness that lasts longer, visible facial blood vessels and burning or stinging can be signs of rosacea. Bumps that resemble acne can appear alongside the redness. These are reasons to seek a diagnosis rather than assuming every red face is simply a reaction to alcohol.6

What does chronic heavy drinking do?

Alcohol metabolism generates reactive oxygen species, molecules that can damage cells when they overwhelm the body's antioxidant defenses. This imbalance is called oxidative stress. In alcohol dependency, it contributes to inflammation, dry skin and accelerated skin aging, including wrinkle formation. Chronic alcohol use also weakens immune defenses and the skin barrier, increasing susceptibility to skin infections.2

Nutrition is another route to skin changes. When alcohol replaces food, or the digestive tract and liver stop processing nutrients properly, deficiencies can develop. Too little protein or too few calories can leave skin dry and less elastic. Keep normal meals in the picture rather than letting the drinks menu write the dinner menu.1

Alcohol can trigger or worsen psoriasis, an inflammatory skin disease, and drinking has been linked with poorer treatment outcomes. The relationship is not identical in every study or person. Alcohol consumption is also associated with increased skin cancer risk, including melanoma, basal cell carcinoma and squamous cell carcinoma.17

What can you do about flushing?

Occasional flushing usually settles without treatment. A cool compress on your face or neck can help you cool down. You can choose a beverage without alcohol, and the choice does not need to become a skincare shopping trip. If you take medication, check its alcohol instructions: some medicines, including metronidazole and certain antibiotics, can cause flushing when combined with alcohol.34

When should you get medical help?

See a dermatologist for a permanently red face or flushing that lasts a long time. Frequent unexplained flushing, or flushing with vomiting or diarrhea, also needs medical assessment. Irritated, bloodshot eyes or red, swollen eyelids can accompany rosacea and require immediate care. Yellow skin or yellow whites of the eyes need medical assessment too: jaundice can occur with liver disease.1456
