Which timeline applies to you?
| What you started | What to expect |
|---|---|
| A topical retinoid, such as adapalene or tretinoin | Acne can worsen for a few weeks before getting better. |
| Retinol for acne | Pimples can continue for the first couple of months, including early worsening. |
| A facial | Post treatment purging should subside within one to two weeks. |
| Acne treatment generally | Fewer breakouts usually take at least six to eight weeks; full effects commonly take two to four months. |
Those timelines describe different things: early worsening, continuing pimples and the eventual treatment benefit. A treatment taking months to reach its full effect does not mean that worsening should continue for months. Look for fewer breakouts as treatment progresses, rather than treating the full treatment window as permission to ignore deterioration.14
Is it purging or irritation?
Purging describes early worsening of breakouts during treatment. Irritation includes dryness, itching, burning, redness and peeling. Retinol can cause both, so the date you started a product cannot tell you which problem you have. Persistent irritation needs a change in approach, not simply more patience.12
A new breakout is not automatically purging. Oily skincare and hair products can cause or worsen acne, and some rashes that look like acne are another condition. If the rash looks unusual, get it checked rather than assuming that every reaction is part of treatment.1
How should you use retinol while your skin adjusts?
Follow your product directions or prescribed plan. Skincare does not have to be complicated, even if a crowded bathroom shelf makes an excellent product catalogue.25
- 1Before first use, test a little retinol on a small area and wait a couple of days for redness or itching.
- 2At bedtime, wash with a gentle cleanser and pat dry. Do not scrub.
- 3Apply a thin layer, about a pea sized amount for the face, avoiding your eyes, mouth and nose. General retinol guidance starts with every other day for the first couple of weeks.
- 4Finish with a moisturizer labelled noncomedogenic, meaning it will not clog pores.
- 5During the day, use broad spectrum, water resistant SPF 30 or higher sunscreen, seek shade and wear protective clothing.
For a prescribed retinoid, ask your dermatologist before changing the amount, frequency or layering. They can recommend less frequent application or moisturizer first to reduce irritation. Applying extra acne medication will not clear acne faster. If you are pregnant or breastfeeding, discuss retinol use with your healthcare provider before using it.24
Should you add exfoliation to speed things up?
Do not add more exfoliation to try to rush the adjustment. Too much acid exfoliation can irritate skin and remove structures that hold water and healthy fats. If glycolic acid is your new product, general guidance is to start once or twice weekly at night and increase only if your skin tolerates it.5
What if the breakout started after a facial?
Follow the home care plan from your facial provider. Avoid booking your first facial immediately before an event where you want predictable skin. Facial add ons are optional, even when the spa menu gives them the confidence of essential services.3
When should you get help?
Contact your healthcare professional if irritation continues despite using treatment less often, acne suddenly worsens, or you develop deep, painful lumps or scarring. Seek a review if over the counter treatment has not helped. Avoid picking or popping spots: that can worsen acne and leave scars.1
