Is it loose skin, fat, or both?

Fullness under the chin can come from extra fat, loose skin, or a combination. Genetics also influence the shape of the jawline. A dermatologist or plastic surgeon can assess which tissues need treatment. Weight loss through healthy eating and physical activity can help when excess weight contributes to chin fat; tightening loose skin is a different job. Jaw and neck exercises build muscle, but stronger muscles do not automatically mean less fat. Avoid overworking them, which can cause strain or jaw joint problems.2

What can skincare do?

Apply moisturizer daily if you want to maintain its temporary plumping effect. Retinol can help the body make more collagen and offer a small improvement, but it cannot replace a lift. Firming creams offer subtle changes at best, despite names that sound like they have a construction permit. Protect your skin from the sun and avoid indoor tanning; these habits also support good results from tightening procedures.1

Which treatment fits your goal?

TreatmentWhat it doesWhat to expect
UltrasoundUses focused energy to stimulate collagen beneath the skin.Best suited to mild to moderate looseness; full effects usually appear in three to six months.
Radiofrequency applied to the skinHeats tissue beneath the surface. Some immediate tightening, with best results around six months; additional sessions can help.
Radiofrequency using needles or a thin tubePlaces heat beneath the skin more precisely.More noticeable tightening, often within about one month, with several days of downtime and wound care.
Neck liftTightens muscles and removes extra skin around the neck and chin.A surgical option for a bigger change, with more recovery than nonsurgical treatment.

How much can ultrasound improve?

Ultrasound can treat the area under the chin without cuts or stitches. One session is often enough, although some people need three or more. Plan for modest improvement rather than a surgical transformation. Patient rated improvements with microfocused ultrasound were mild, and longer term results beyond one year remain uncertain. Treatment can be moderately painful. Temporary redness, swelling, bruising and tenderness can occur; less common complications include burns and altered skin sensation.34

When does surgery make more sense?

Substantial sagging is less likely to respond well to nonsurgical tightening. A neck lift removes excess skin and tightens supporting muscles; liposuction removes fat instead. Fat dissolving injections target small amounts of fat where the skin is firm, not loose skin. A facelift can also address loose neck skin and extra fat, because cosmetic procedure names are not reliable maps.125

Before choosing surgery, discuss the exact technique, likely outcome, anesthesia, risks and recovery with your plastic surgeon. Share your medical conditions, medicines, supplements and smoking history. Arrange transport home and someone to stay with you for at least the first 48 hours after a facelift. A traditional facelift can require up to three weeks off work.5

Who should be careful with tightening procedures?

Do not book a skin tightening procedure during pregnancy or with an active skin infection. Certain medicines also make treatment unsuitable. If you are breastfeeding or have an autoimmune condition, discuss ultrasound with your healthcare provider first. Choose a board certified dermatologist and a practitioner experienced in the specific treatment. Ask about expected improvement, side effects, recovery and the full cost before committing.13
