What do closed comedones look like?

A closed comedo is a plugged follicle whose opening is covered by skin. Closed comedones are also called whiteheads, but they often match the surrounding skin rather than looking bright white. Whitehead is the name, not a promise that every bump will arrive in white. They are uninflamed bumps, unlike swollen pimples. A white or yellow centre containing pus belongs to a different type of acne, called a pustule.245

Acne can include several types of spots at once, so closed comedones can sit alongside blackheads or inflamed pimples. Blackheads have an open pore, while closed comedones have a covered opening. Deep, painful lumps are not the same as small, uninflamed whiteheads and need a different treatment plan. If you cannot tell which kind of bump you have, a clinician can examine your skin.125

Why do they appear on the chin?

The chin is a common site for comedonal acne, so the location is not much of a detective. The blockage forms when sebum, the skin's natural oil, and dead skin cells collect inside a pore or hair follicle. Increased oil production, hormones and the way skin forms keratin, a structural protein, can contribute.5

Acne has several contributing factors rather than one universal cause. Family history, hormones, certain medicines, high humidity and cosmetics that block follicles can all play a role. Hormone changes during puberty, periods, menopause and pregnancy can contribute to acne, but chin bumps alone are not a reason to assume a hormonal disorder. Most acne does not need testing; unusual features can prompt a clinician to consider further checks.36

Which treatments help?

TreatmentRole in acne careWhen to consider it
Topical retinoidsRetinoids help unclog pores. Examples include adapalene and tretinoin.A treatment to discuss for whiteheads and blackheads.
Salicylic acidAn acne treatment available in products applied to the skin, including washes. An option to discuss with a pharmacist.
Benzoyl peroxideHelps reduce acne bacteria; a wash can accompany a retinoid for whiteheads and blackheads.Useful to discuss when choosing an acne routine, including one for mixed breakouts.
Azelaic acidA topical treatment used for mild acne.Another treatment option to discuss if you need a different plan.
26

For closed comedones, a retinoid is a direct way to address the blocked pore. Adapalene is available without a prescription in some places; local availability differs. A benzoyl peroxide wash has a different job, reducing acne bacteria, rather than replacing the retinoid's role. A pharmacist can help choose an acne product, and a clinician can consider prescription treatment if shop bought options are not helping.23

How should you care for your chin?

  1. 1Wash your face gently with a gentle cleanser in the morning and evening. Do not scrub the chin to try to remove the bumps.
  2. 2Use the acne treatment chosen with your pharmacist or clinician. Prescription topical medicines can come as creams, gels or lotions.
  3. 3Moisturize your skin morning and night. Choose a moisturizer intended not to clog pores.
  4. 4Wear sunscreen daily to protect your skin from sun damage.
  5. 5Remove makeup at the end of the day rather than sleeping in it.
35

Choose oil free, water based, noncomedogenic cleansers, moisturizers and makeup. Noncomedogenic means intended not to clog pores, which is a more useful label than a promise of 'perfect skin'. Greasy skin or hair products can contribute to acne, so consider the products touching the affected area as well as the treatment you put on it.35

How long does treatment take?

Give whitehead treatment six to eight weeks before judging whether it is helping. Comedonal acne can start improving in eight to twelve weeks, while clearing can take several months. These are different checkpoints: an early review is not the same as complete clearance. If blemishes remain after six to eight weeks, a dermatologist can assess whether you need prescription treatment or professional extraction.25

Should you squeeze or extract them?

Do not pick, squeeze or puncture closed comedones at home. Picking whiteheads can lead to more whiteheads and scars. Closed comedones have a covered opening, and professional extraction uses sterile instruments to remove the clog. Trying to do that yourself can damage follicles, cause infection or turn the bump into inflammatory acne. Extraction is an option for stubborn clogs, not a substitute for choosing an ongoing treatment plan.15

When should you get medical help?

  • See a clinician if treatment is not improving the bumps or you have many spots.
  • Get assessed for deep, painful lumps or acne that is causing scars rather than continuing to treat everything as whiteheads.
  • Seek medical help for hot, swollen or red skin that could indicate infection. Redness can be harder to see on brown and black skin.
  • Tell your clinician about medication side effects so the treatment plan can be reviewed.
  • Ask for help if acne is affecting your mental health.
35

A clinician can offer prescription topical treatment or refer you to a dermatologist. Persistent acne sometimes needs oral medicine, but the choice depends on the acne pattern and response to treatment. Closed comedones can recur over years, so ongoing skincare matters even after the chin becomes clearer.356
