Why does arm skin sag?

Loose skin has lost elasticity and firmness. Aging reduces collagen and elastin, the substances that help skin stay firm and elastic. Genetics also affect how skin responds to aging and weight changes. Significant, rapid weight loss can leave skin unable to adapt to the smaller body underneath. The longer someone has been overweight, the less likely their skin is to bounce back. Sagging can range from mild wrinkling to noticeable folds. After substantial weight loss, upper arms are a common place for excess skin, and loss of muscle mass can accompany the change. Thin people can also have loose upper arm skin because of previous weight loss, aging or genetics.245

How to choose your next step

  1. 1Decide what you want to change: muscle definition, dry skin or actual hanging skin. These need different approaches.
  2. 2For muscle definition, use resistance training. For dry skin, apply moisturizer daily. Neither removes excess skin.
  3. 3If sagging is slight, discuss procedures with a board certified dermatologist. These work best with a small amount of loose skin and a healthy, stable weight.
  4. 4If you have substantial hanging skin, arrange a plastic surgery consultation to discuss an arm lift, its scar and recovery.
  5. 5Before committing, ask about the expected improvement, total cost, number of treatments and time away from normal activities.

Can exercise tighten loose arm skin?

Weight training builds muscle underneath loose skin and can sometimes improve its appearance. Exercise improves muscle tone, but it cannot remove excess skin, however enthusiastically a workout is named. More weight loss is not a treatment for hanging skin and can add to the excess. If you are considering surgery, a relatively stable weight matters more than trying to lose additional weight specifically to tighten your arms. Maintaining weight and general fitness also helps preserve surgical results.24

Do firming creams or retinol help?

A firming cream can moisturize skin, but the word "firming" is not a surgical qualification. Moisturizer temporarily plumps skin; apply it daily to maintain that benefit. Retinol can support collagen production, with subtle improvement at best. Neither lifts hanging skin. Products containing collagen or vitamin E can support hydration, but they cannot eliminate excess skin. Treat moisturizing as care for dryness and appearance, not a substitute for removal.14

Which procedures can tighten arm skin?

OptionWhat it doesTiming and tradeoffs
UltrasoundHeats deeper skin to stimulate collagen.Modest tightening in 2 to 6 months after one treatment.
Radiofrequency applied to the skinHeats tissue beneath intact skin.Best results around 6 months; some people need further treatment.
Laser without surface injuryHeats deeper skin; used for loose upper arm skin.Usually 3 to 5 treatments; results 2 to 6 months after the last.
Radiofrequency through tubes or needlesDelivers heat beneath the skin; used on upper arms. Results around one month; sedation, wound care and several days of downtime.
Arm liftRemoves excess skin and tightens supportive tissue.Immediate shape change; permanent scar and surgical recovery.

Choose a procedure without surgery for modest tightening, not removal of large folds. Pregnancy, skin infection and certain medications rule out skin tightening procedures. Discuss prescriptions before booking. Treatments that leave skin intact usually involve temporary redness or swelling and little downtime; costs vary and insurance does not cover them.1

What does an arm lift involve?

An arm lift, called brachioplasty, removes excess skin and tightens the tissue supporting it. Liposuction can be added to remove fat deposits; removing fat and removing loose skin are separate jobs. The incision sits on the inside or back of the upper arm. Depending on the amount of excess skin, it can extend from the underarm to just above the elbow, or onto the side of the chest. Surgery is generally outpatient and typically takes one to two hours, sometimes longer with additional procedures. Good candidates have significant excess skin, relatively stable weight and no conditions that impair healing or increase surgical risk. Nonsmoking and accepting a lasting scar are important. Ask the surgeon about their training and experience with arm lifts. Discuss medical conditions, allergies, prescriptions, vitamins and herbal supplements during the consultation. Get specific preparation instructions rather than changing medicines yourself.23

How long does arm lift recovery take?

Arm shape changes immediately, but swelling and bruising take weeks to settle. Dressings protect the incision, and a compression sleeve is worn for several weeks. Small drainage tubes are sometimes needed. Recovery includes wound care, rest, arm elevation and avoiding strenuous exercise or heavy lifting until cleared. Scars can fade but never disappear completely. Results can last a long time with stable weight, although aging and major weight changes can bring back looseness. Surgical risks include bleeding, infection, fluid buildup, altered sensation, tissue damage and wounds separating.3

What helps while you decide?

Compression garments can help manage the appearance of loose skin, which is a clothing solution rather than a disappearing act. Loose clothing can reduce rubbing and discomfort. Keep folds clean and dry because trapped moisture and difficult cleaning can contribute to soreness or infection. After bariatric surgery, follow your care team's protein and vitamin plan and attend nutritional checkups. Rapid weight loss can cause dry skin as well as changes in body shape. Moisturizer can address dryness, but nutrition care is not a way to remove excess skin. If loose skin causes emotional distress, your bariatric team or GP can help you access psychological support.456

When should you contact a doctor?

See your GP if folds become sore, repeatedly infected or a major problem. NHS surgery for loose skin is generally not funded unless there is a significant medical need or exceptional circumstances. Cosmetic arm lifts are also usually excluded from insurance, although recurring infections can affect coverage decisions. After an arm lift, contact your surgeon for abnormal swelling, excessive bleeding, fever, discharge, uncontrolled pain or changes in arm or hand sensation or movement. Chest pain or shortness of breath also needs immediate medical attention.36
