How do hormones cause back acne?
Hormonal changes can increase sebum, the oil your skin produces. Excess oil and dead skin cells block hair follicles, the openings where hairs grow. Bacteria in blocked follicles can then cause inflammation and pus. Hormonal acne can affect the back, shoulders and chest as well as the face, and can include blackheads, whiteheads and painful cysts. The jawline does not have exclusive rights to hormonal acne.12
What makes a hormonal contribution more likely?
These patterns are worth discussing with a doctor:
- Breakouts that flare before your period. Premenstrual flares are common in adult acne.
- Acne that starts or changes during puberty, pregnancy or menopause.
- New acne after stopping estrogen containing birth control or starting testosterone treatment.
- Acne alongside irregular periods or excessive hair growth, which can accompany a hormonal disorder.
A doctor assesses the breakouts alongside your health history, skincare, stress, sleep and current medicines. Mention medicines you have recently stopped as well. Corticosteroids, lithium and some seizure medicines can contribute to acne. Adult acne can also persist from adolescence or first begin after age 25, so a late arrival is not unusual.134
What else can contribute to back acne?
Greasy hair and skin products can block follicles. Sweaty clothing and friction from a backpack can aggravate back acne, while stress can worsen existing acne. Hormones and these everyday triggers can contribute at the same time. Pimples are not caused by dirt, so back acne is not an invitation to turn showering into a cleaning inspection.456
How should you treat it?
You do not need to establish a hormonal cause before treating mild back acne. A few recent spots, or blackheads and whiteheads without deep pain, can be suitable for home treatment. Hormonal acne can still respond to treatments applied to the skin.17
- 1Wash gently. Avoid abrasive scrubs, loofahs and back brushes, which can worsen acne.
- 2Try a benzoyl peroxide foaming wash daily. Leave it on your back for two to five minutes, then rinse thoroughly. Benzoyl peroxide can bleach fabrics. A 5.3% wash is less likely to cause irritation, dryness or peeling than a stronger wash.
- 3Consider adding adapalene 0.1% gel daily after showering or before bed. This retinoid helps unclog pores and can improve results alongside benzoyl peroxide. A back lotion applicator can help you reach the affected area.
Wear loose, breathable clothes, change out of sweaty clothing and shower after exercise. Wash workout clothes after each use and avoid bags that rub your back. Choose gentle, fragrance free skincare and products labelled noncomedogenic, meaning designed not to clog pores. Do not pick or squeeze spots because this can cause scars.27
If you are pregnant, ask your healthcare provider which acne treatment is safe before starting. Avoid topical retinoids such as adapalene during pregnancy. Oral isotretinoin must not be taken during pregnancy because it can cause birth defects.13
When should you see a doctor?
Back acne treatment can start showing results in six to eight weeks; complete clearing can take three to four months. See a dermatologist if there is no improvement after six to eight weeks. Get help sooner for deep, painful lumps, scarring, hot or swollen skin, or acne affecting your mental health.27
For widespread acne or breakouts that resist topical treatment, a doctor can consider oral medicines. Options include antibiotics, a combined contraceptive pill when appropriate, or isotretinoin. If hormones are contributing, certain contraceptive pills or spironolactone can be used. Treatment depends on severity and your medical circumstances, not simply where the spots appear.45
