What actually happens during purging?
Retinoids are vitamin A based treatments that help prevent clogged pores and speed the turnover of surface skin cells. Tretinoin increases production of new skin cells and unclogs pores. Early worsening can occur, but the explanation is not always simply that existing blockages are coming to the surface: ongoing acne and a delay before treatment starts working can also account for more spots.145
With tretinoin 0.025% gel alone, 15.4% of people with mild acne had at least a 10% increase in inflamed spots after two weeks, compared with 8.7% using the gel without the medication. The same pattern did not occur in moderate or severe acne. Early flaring depends on the treatment and situation; it is not something every retinoid user must experience.4
Is it purging, irritation or another breakout?
A breakout after a new product is not automatically purging, even if the word gives the product a convenient alibi. Cosmetics and skin products can contribute to acne, and several conditions can look like acne without being acne. The product you started, the symptoms you have and how the problem develops all matter more than the name you give it.36
Burning, stinging, peeling and dryness are recognised side effects of topical acne treatments. Excessive application can make irritation worse. These symptoms deserve attention rather than being treated as proof that the product is clearing your pores. Irritation from a retinoid does not usually mean an allergy, but a dermatologist can help distinguish the two.36
How long should early worsening last?
For tretinoin, acne can worsen during the first 7 to 10 days. Acne sores should begin improving in 2 to 3 weeks, though improvement sometimes takes more than 6 weeks. Breakouts described as purging after a facial should subside within a week or two. Avoid scheduling your first facial immediately before an event where you want predictable skin.12
Acne treatment generally takes at least 6 to 8 weeks before you start seeing fewer breakouts. That is a treatment response timeline, not a rule that every worsening breakout should be endured for eight weeks. Acne treatment aims to stop new breakouts, so a worse before photo is not a required entry fee.6
How should you handle a new treatment?
- 1Follow the treatment's directions. For prescribed tretinoin, wash with a mild cleanser, let your skin dry as directed and apply a thin layer at bedtime. Do not use extra medication to speed up results.
- 2Start cosmetic retinol gently. General guidance is to choose a low strength and begin every other night, increasing gradually. For a prescription retinoid, ask your dermatologist before changing the amount or frequency.
- 3Use moisturizer to help manage dryness. Your dermatologist can recommend applying moisturizer before a retinoid if irritation is a problem.
- 4Avoid adding more exfoliation to chase the breakouts. Too much acid exfoliation can strip away structures that retain water and healthy fats. Ask your prescriber before combining tretinoin with other topical medications.
- 5Protect your skin during the day. Use shade, protective clothing and a broad spectrum, water resistant sunscreen with SPF 30 or higher.
When should you get medical advice?
With tretinoin, call your doctor immediately for rash, hives or pain at the treated area. Report severe or persistent burning, swelling, blistering or peeling. Seek assessment for acne that is causing scars or continuing despite treatment rather than repeatedly labelling it purging.13
Retinoids should not be used during pregnancy. If you have very dry, sensitive or already inflamed skin, discuss suitability before starting. In darker skin, irritation can trigger dark marks, making a gradual introduction and moisturizer particularly useful.5
