Why drying your skin is not the answer

Acne starts when oil and dead skin cells block pores. Bacteria trapped inside can trigger inflammation. Drying the surface with witch hazel is not the same as clearing those blockages or treating the bacteria. Astringents, products used to dry or tighten skin, can cause irritation that makes acne look worse.34

Choose gentle skincare labelled alcohol free, especially if your skin is sensitive or already irritated. Alcohol free is a useful selection rule, not a reason to treat witch hazel as acne medicine. A toner is a product format, not an acne treatment credential, however official the bottle looks.14

What about acne routines containing witch hazel?

A six week trial in 30 people with mild to moderate acne found improvement with a morning and evening routine containing a 3% sulfur cleanser, a toner containing witch hazel and glycolic acid, and a 5% benzoyl peroxide treatment. Improvements appeared after two weeks and continued through week six. Witch hazel's individual contribution remains unknown, so that result is not a reason to replace an acne treatment with plain witch hazel.5

What should you use instead?

  • For an occasional inflamed pimple, benzoyl peroxide kills bacteria involved in acne. Products come in strengths from 2.5% to 10%; choose a lower concentration if your skin is sensitive.
  • For mild clogged pores and small bumps that are not inflamed, salicylic acid helps unclog pores and exfoliate skin. It does not kill acne bacteria.
  • For persistent acne, other treatment options include topical retinoids, medicines related to vitamin A, and azelaic acid. A pharmacist or doctor can help you choose an appropriate treatment.

How to build a routine without witch hazel

A routine can include cleanser, acne treatment and moisturizer without a witch hazel step, so the toner does not need a ceremonial place in the queue.6

  1. 1Gently wash with a mild cleanser in the morning and evening, and after sweating. Use your fingertips rather than scrubbing tools.
  2. 2Apply your chosen acne medicine sparingly and follow its label directions. There is no single application schedule that fits every cleanser, cream and gel.
  3. 3If a treatment leaves skin dry or tight, layer a fragrance free moisturizer over it. Look for noncomedogenic on the label, meaning less likely to clog pores.
  4. 4When outdoors, finish with a broad spectrum, water resistant sunscreen with SPF 30 or higher. Choose one labelled noncomedogenic.

How long should you wait for improvement?

Acne treatments often take six to eight weeks to show a response, and improvement can take several weeks to a few months. Switching treatments repeatedly can irritate skin and cause more breakouts. If you suspect witch hazel is contributing to breakouts, remove that product for two weeks while leaving other products unchanged; fewer breakouts during that period can help identify a possible trigger.346

When should you stop or get help?

Stop using witch hazel if it causes irritation rather than repeatedly applying it to dry out a spot. See a doctor if acne is deep, painful, causing scars or affecting your mental health, or if self treatment is not helping. Hot, swollen skin with redness can indicate infection and also needs medical assessment. Do not pick or squeeze pimples, because that increases the risk of scarring.12
