The distinction is the type of mark: this advice concerns dark patches left after pimples, not red marks or indented scars. Those dark patches are called post inflammatory hyperpigmentation, meaning extra pigment after inflammation. They are especially common in darker skin tones.12

Azelaic acid is used for mild to moderate acne, including blackheads, whiteheads and inflamed pimples. The word "acid" is not a treatment ranking, however serious it looks on a bottle. The useful distinction is what the ingredient treats, not its name.3

How should you use azelaic acid?

  1. 1Choose one treatment to start, rather than turning your bathroom shelf into a selection committee. Follow the product's directions instead of adding several acne treatments at once.
  2. 2Cleanse your skin before applying azelaic acid cream. General cream guidance is to start once daily and increase to twice daily if tolerated; follow your own product's instructions.
  3. 3If you are treating ongoing acne, cover the affected acne prone area rather than only individual pimples. Treating the wider area helps address existing acne and prevent the next breakout.
  4. 4Before going outside, apply tinted sunscreen with iron oxide, SPF 30 or higher, broad spectrum protection and water resistance. Reapply every two hours outdoors and after swimming or sweating.

Azelaic acid preparations include 20% creams and lotions and 15% gels. Approved uses differ by country and preparation, so a gel's directions are not automatically a cream's directions. There is no specific niacinamide strength, texture or price advantage established for this comparison.3

How long will the marks take to fade?

Acne can start improving after about a month of azelaic acid cream, with further improvement over six months of continuous use. That is an acne timeline, not a deadline for the marks. Once their cause stops, spots a few shades darker than your skin usually fade within 6 to 12 months; deeper pigment can take years. Treatment can speed fading. Controlling ongoing acne matters because new breakouts can leave new marks.34

Who should be careful?

Very sensitive skin or eczema can make azelaic acid irritating. Avoid it if you are allergic to azelaic acid or another ingredient in the formula. Skin lightening can occur and is more apparent in darker skin tones. Stop and seek medical advice for severe redness, scaling, itching or burning, or if asthma worsens during use.3

If you are pregnant or trying to become pregnant, consult a dermatologist before treating acne yourself. Get help for deep, painful pimples, acne that leaves scars, or persistent breakouts and marks despite treatment. If discoloration begins while taking medication, ask the prescriber about it rather than assuming every mark comes from acne; some medicines cause pigmentation changes. Do not stop prescribed medication yourself.145
