What can the bumps tell you?
The type of bump is more useful than its location alone. Small bumps that match your skin colour and are not tender or swollen can be comedonal acne, meaning clogged pores. This commonly affects the forehead and chin. Closed plugs form whiteheads; open plugs form blackheads. Blackheads darken through contact with air, not dirt.14
Inflamed bumps can look pink or red on white skin, or brown or purple on brown and black skin. Bumps topped with pus are pustules. More than one type of acne can appear together, so a forehead breakout does not necessarily have one uniform appearance.5
Why is acne appearing on your forehead?
Start with what touches the area. Shampoos, conditioners and styling products can cause closely packed whiteheads or tiny bumps along the hairline and upper forehead. Hair product oils can reach the skin and clog pores, even if you have never had acne before. The bathroom shelf has separate hair and skin sections, but the products do not respect the seating plan.6
Hormone changes during puberty and pregnancy can increase acne risk, and family history also matters. Certain medicines, including corticosteroids, lithium and medicines containing hormones, can contribute. Pressure from sports helmets, humidity and harsh scrubbing can worsen acne. Stress can make existing acne worse, but stress and dirty skin are not causes of acne. Some foods worsen acne in some people; forehead location alone is not a reason to blame a particular food.17
What should you do first?
- 1Review hair products touching your forehead. Stop using suspected pore clogging products. Look for labels such as "oil free", "noncomedogenic", "nonacnegenic" or "won't clog pores".
- 2Remove leftover product residue. Wash pillowcases, sheets, hats, caps and headbands that touch your head. Product residue can stay on these items, giving a discontinued product an unofficial forwarding address.
- 3Wash gently morning and night, and after sweating. Use lukewarm water and your fingers rather than scrubbing with a washcloth or sponge. Remove makeup before bed.
- 4Choose noncomedogenic skincare and makeup, meaning products designed not to clog pores. Use an oil free moisturizer if your skin feels dry, and wear sunscreen daily.
- 5For mild acne, ask a pharmacist about a topical treatment such as salicylic acid or benzoyl peroxide. Do not pick or squeeze spots.
How long should clearing take?
Breakouts caused by hair products can take four to six weeks to clear after the offending products stop touching your skin. If they have not cleared by six weeks, see a dermatologist. Comedonal acne can take eight to twelve weeks to start improving with consistent treatment, and many months to clear. Removing a product from the shelf is immediate; removing its contribution to the breakout is not an express service.46
What does cystic acne on the forehead mean?
Cystic acne involves painful, pus filled bumps deep under the skin. Nodules are also deep and painful, but are solid rather than filled with fluid. Both can scar. See a dermatologist rather than relying on shop bought acne products for these lumps. Treatment often involves prescription creams or gels and oral medicines, and clearing cysts can take three months or longer.3
Get medical help for deep or painful spots, scarring, acne that is not improving with self treatment, or acne affecting your mental health. Hot, swollen, red skin can signal infection; redness can be harder to see on brown and black skin. Report medication side effects to your healthcare provider.45
