What do the spots look like?
To describe the breakout, look for these ordinary acne features:
- Whiteheads are small, skin coloured bumps.
- Blackheads are black or brown spots, usually flat but sometimes raised.
- Papules are raised bumps that look pink or red on white skin and can look brown or purple on brown and black skin.
- Pustules are raised bumps filled with pus.
These descriptions identify the spots, not their cause. Acne develops when hair follicles become blocked with oil and dead skin cells. Tretinoin increases production of new skin cells and unclogs pores; exfoliating acids speed the removal of dead surface cells. A change in those processes gives useful context, but it does not make every new breakout a purge.245
Is it purging or irritation?
Look at what changed besides the number of spots. Dryness, redness and peeling are recognised retinoid side effects. Too much exfoliation can also irritate skin. These symptoms can occur alongside acne, so extra pimples and irritated skin are not mutually exclusive. Write down which product you started and when, rather than asking the bathroom shelf to keep records.56
How long should it last?
The timing depends on the treatment. Breakouts after a facial should subside within one to two weeks. With tretinoin, acne probably gets worse during the first seven to ten days, with more acne sores and red, scaling skin. The sores should improve after two to three weeks of regular use, although sometimes improvement takes more than six weeks. Those are treatment specific timelines, not a universal countdown for every serum.12
Early worsening and the time needed to control acne are different things. Acne treatment generally takes at least six to eight weeks before fewer breakouts appear, and topical retinoids can take twelve weeks or longer to show improvement. Persistent breakouts need assessment rather than an indefinite extension of the purging explanation.367
What should you do while spots settle?
- Wash gently up to twice daily and after sweating, using your fingertips and a gentle cleanser rather than scrubbing tools.
- Avoid adding irritating toners, astringents or extra exfoliants. Picking and squeezing make spots take longer to clear and increase the risk of scars and dark marks.
- Use medication exactly as directed. Tretinoin is used at bedtime; apply a thin layer to clean skin dried as directed. Do not add other topical medicines without your doctor's approval.
- For retinoid irritation, ask your dermatologist about a smaller amount, less frequent use or moisturizer before medication. Make prescription changes with their help.
- Protect exposed skin outdoors with broad spectrum, water resistant sunscreen rated SPF 30 or higher, alongside shade and protective clothing.
Give acne treatment time to work and keep your routine consistent rather than turning the bathroom shelf into a rolling product audition.7
When should you get medical help?
Seek medical help for deep, painful lumps, scarring, breakouts that are not improving, or hot, swollen skin suggesting infection. With tretinoin, call your doctor immediately for rash, hives, itching, or pain at the treated area; report severe or persistent burning, swelling or blistering. Do not keep pushing through these symptoms as purging. If you are pregnant, planning pregnancy or breastfeeding, tell your prescriber before using tretinoin; call them if you become pregnant during treatment.24
