Which moisturizer should you choose?

TypeWhen it fitsWhat to know
CreamDaytime useAbsorbs quickly and is less greasy than ointment.
OintmentVery dry, thickened skin or nighttime useThick, greasy and very moisturizing. Do not use on weeping eczema.
LotionOrdinary chapped handsThin and less moisturizing. Avoid for dyshidrotic eczema.

For ordinary chapping, a lotion can help; switch to a cream or ointment if it is not enough. For hand eczema, start with a thick, fragrance free cream or ointment. Emollient is simply the medical name for a moisturizing treatment.1345

Avoid aqueous cream as a leave on moisturizer: its sodium lauryl sulphate can irritate skin.3

How to wash and moisturize your hands

  1. 1Remove rings before washing or applying moisturizer. Moisture trapped underneath can trigger blistering hand eczema.
  2. 2Wash gently with lukewarm water and a mild, fragrance free cleanser. Avoid antibacterial and deodorant soaps if you have dyshidrotic eczema.
  3. 3Rinse and pat dry rather than rubbing. Smooth moisturizer gently over the skin.
  4. 4Reapply after each wash and whenever your hands feel dry. Aim for at least three or four applications daily, with more whenever needed.
13

Keep moisturizer at home and at work so repeated application does not become a search operation. Apply a thin layer to affected areas before work, then reapply after washing or when dryness returns. Check whether oily products interfere with your work, particularly tasks needing fine hand control or handling paint.35

Can you use a soap substitute or hand sanitizer?

An emollient soap substitute can clean dry skin without ordinary soap's drying effect. Soap substitutes do not foam like ordinary soap, so the missing bubbles are not a performance review. Mix a little with warm water, spread over the skin, rinse and pat dry.23

Skin cleansing and infection control are different jobs. Cream cleansers are not antimicrobial. In healthcare settings, alcohol hand rubs cause less irritation than repeated soap and water washing and reduce disease transmission. Choose a fragrance free, colourless alcohol cleanser; use soap and water when hands are visibly dirty or contaminated with blood or other body fluids. Follow workplace hygiene requirements.6

For everyday chapped hands, avoid unnecessary washing while maintaining good hygiene. Avoid hot water and protect your hands from extreme cold and wind. If a sanitizer triggers your dyshidrotic eczema, ask a dermatologist about a suitable alternative; sensitivity to an ingredient can cause a flare.14

Which gloves help?

For dishwashing, cleaning and other wet work, wear cotton gloves underneath waterproof gloves. Cotton gloves alone suit tasks where your hands stay dry. Put gloves on before contact with water, detergents or household cleaners, rather than after your hands start feeling irritated.1

Keep gloves clean and dry inside. Sweating in rubber or plastic gloves can aggravate eczema, so avoid prolonged wear where possible. Choose protection for the actual chemical or task: no glove material protects against everything indefinitely, however official the box looks.256

For healthcare workers wearing enclosed gloves for more than 20 minutes, cotton liners reduce sweat irritation. Nitrile is an alternative for latex allergy. Avoid petroleum based emollients under latex gloves because they weaken the rubber. Workplace glove requirements take priority over choosing a material solely for comfort.6

Is it dryness, eczema or a contact reaction?

Hand eczema can cause itching, scaling, warmth, swelling and cracks. Atopic dermatitis, a form of eczema, can affect only the hands in adults. On darker skin, affected areas can look dark brown, purple or gray rather than red. Repeated scratching can leave skin thickened and persistently itchy.7

Repeated water exposure, detergents, solvents and friction can damage the skin's protective barrier. Allergic contact dermatitis is different: an allergic reaction can appear hours or days after contact with substances such as fragrance, metals, rubber ingredients or preservatives. More than one cause can contribute to hand eczema.25

Dyshidrotic eczema is the blistering form that affects fingers and palms. Heat, sweating and dryness can trigger it. Avoid overheating, moisturize frequently and try not to scratch. A dermatologist can advise whether a barrier repair cream containing dimethicone would help protect your skin from irritation.15

When do you need prescription treatment?

Prescribed steroid creams or ointments calm inflamed hand eczema. Thick palm skin usually needs stronger treatment than a mild steroid, so do not keep escalating products yourself. Use the strength and course your clinician specifies. Overuse can thin skin; correctly directed use does not usually cause this problem.2

Leave 20 to 30 minutes between moisturizer and a steroid or other skin treatment. Ask your doctor which to apply first.3

Other prescription options include calcineurin inhibitor creams, which do not thin skin but can burn or sting. Severe eczema sometimes needs specialist light treatment or medicines taken by mouth. Alitretinoin is a specialist option for severe, longstanding hand eczema and must never be taken during pregnancy. Antihistamines are not generally helpful for hand eczema; sedating ones sometimes help sleep but cause drowsiness.2

How long does improvement take?

Hand eczema can be brief or persist for years. Treatment usually controls the condition rather than curing it; avoiding a confirmed allergen can sometimes clear it. Continued irritant exposure can prolong flares for months, so protection matters alongside cream. Contact a healthcare professional if your hands are not improving.245

Persistent, recurrent or severe hand eczema warrants dermatology assessment, particularly if it interferes with work or everyday tasks. Patch testing checks for delayed contact allergies. A persistent rash on one hand also needs assessment because fungal infection and other skin conditions can resemble eczema.25

What should you stop for or get checked?

If a wash product stings after rinsing, ask a pharmacist for an alternative. Get advice for persistent burning or stinging from moisturizer. Use a clean spoon for tubs. Keep emollient contaminated clothing and bedding away from flames and cigarettes; washing does not completely remove the fire risk.3

Contact a clinician for yellow or golden crusts, pus, increasing pain or spreading inflammation, which can indicate infection. Seek urgent medical care for blisters accompanied by fever or feeling unwell: eczema can develop a serious herpes infection. This is especially urgent for children and people with weakened immune systems.57
