Is it swelling, an eye bag or a wrinkle?

Temporary puffiness happens when fluid builds up around the eyes. A persistent bag can instead come from the normal fat behind the eye moving forward. Excess skin also contributes to bags and wrinkles, while hollows beneath bags can create shadows. These need different treatments, despite being assigned to the same tiny jar.14

The skin around the eyelids is particularly thin, constantly moves with blinking and is exposed to sunlight. Over time, that combination contributes to stretching and sagging. A heavy upper eyelid can involve the eyebrow position, extra skin or the eyelid itself; treating one does not automatically address the others.4

How to reduce temporary puffiness

  1. 1Lie down and place a washcloth soaked in cold water across your closed eyes for a few minutes.
  2. 2If you use an ice pack instead, wrap it in a towel to protect your skin.
  3. 3Consider a caffeine eye cream for cosmetic puffiness, following its directions.
  4. 4If your eyes or eyelids swell while wearing contacts, remove the lenses immediately.

For recurring puffiness, aim for seven to nine hours of sleep, raise your head slightly with another pillow and limit salty foods. Antihistamine eye drops help only when allergies are the cause. Use steroid eye drops only as prescribed.1

How to build a wrinkle routine

  1. 1Test a new skincare product on your inner forearm twice daily for four to five days before using it on your face. Even a product labelled "hypoallergenic" needs testing.
  2. 2Use moisturizer to help soften the appearance of fine lines.
  3. 3Apply broad spectrum sunscreen with SPF 30 or higher every day to exposed skin.
  4. 4If retinol suits your skin, introduce the least intense formula you can find every other night, increasing use slowly as tolerated.
  5. 5Follow the product's directions for amount and placement around the eyes. Do not assume a facial product is intended for the eyelids.

Retinoids are vitamin A based ingredients; retinol is one member of that family and is available without a prescription. Retinoid treatments can improve fine lines and skin texture. Tretinoin increases surface skin cell turnover and boosts collagen, the protein that helps support skin. Prescription tretinoin is not the starting point everyone needs.3

There is no single retinol percentage to choose for everyone. Start with the lowest formulation you can find; over the counter products do not have a standard requirement to disclose their retinol percentage. Use retinoids at night and protect your skin from sunlight during the day. Moisturizer helps reduce irritation during introduction.3

Who should be careful with retinol?

Do not use retinoids during pregnancy. Skin allergies, dryness, substantial redness or inflammation are reasons to discuss alternatives with a dermatologist before starting. Irritation can trigger dark marks in darker skin, so slow introduction matters. For sensitive skin, consider sunscreen containing zinc oxide or titanium dioxide.3

How long will results take?

Moisturizer can immediately plump fine lines. Most other wrinkle products take at least six weeks to work, sometimes three months. Avoid adding several wrinkle treatments while you wait: irritation makes signs of aging more noticeable.2

What should you stop using?

Stop skincare that stings or burns unless a dermatologist prescribed it and is supervising its use. Do not use hemorrhoid cream around your eyes. If eye cosmetics or creams seem to trigger swelling, leave them out for at least a couple of weeks. Eye swelling lasting longer than 24 to 48 hours needs an eye care professional.12

What works for persistent bags and deeper wrinkles?

An ophthalmologist or oculofacial plastic surgeon can examine the eyes, eyelids, lashes and surrounding skin before discussing treatment. Options include Botox injections, fillers, laser resurfacing, radiofrequency tightening and eyelid surgery. Blepharoplasty, the medical name for eyelid surgery, can reduce lower eyelid bagginess and remove excess upper eyelid skin.5

Botox targets muscle activity that creates wrinkles such as crow's feet. Fillers add volume to hollows rather than removing a bag. Under eye fillers can cause fluid buildup or a bluish appearance, and rarely permanent vision loss. People prone to eyelid fluid buildup are not always suitable candidates.4

Surgery can remove or reposition fat as well as address excess skin. Removing fat alone can deepen the hollow below a bag; repositioning it can address both areas. Recovery involves a greater commitment than injections. Eyelid exercises do not prevent drooping, so blinking does not need a training plan.4

What can you use for wrinkles on your face?

For facial wrinkles beyond the eye area, treatment depends on the type of line. Botox blocks signals that make muscles contract, so it targets movement lines such as frown lines. Wrinkles that remain when your face is at rest can need a different approach, including fillers for deeper folds.6

Other clinical options include microneedling, which creates tiny wounds to stimulate collagen and elastin, chemical peels and laser resurfacing. Lasers can treat fine lines around the eyes, forehead or mouth. Certain laser treatments carry a higher risk of darkening skin in people with darker skin tones. Before a procedure, discuss your medical history, allergies and goals with the clinician so the treatment fits the concern.6
